Abstract/Summary:
Objective:To investigate the current situation of benefit finding and symptom burden and communication pattern of marrige in patients with cervical cancer,to analyze the influencing factors of benefit finding in patients with cervical cancer,and discuss the relationship between the above three variables and their action paths,to examine the intermediary role of communication pattern of marrige between symptom burden and benefit finding,in order to provide reference for targeted intervention of medical staff.Methods:This study uses the method of cross-sectional investigation.From February to June,2022,cervical patients who met the inclusion criteria were selected by convenient sampling method in the Oncology ward and outpatient department of four Grade-A hospitals in Shandong Province.Benefit Finding Scale(BFS),M.D Anderson Symptom Inventory(MDASI)and Communication Pattern Questionnaire(CPQ)to investigate the patients with cervical cancer disease.Excel was used to input data,and SPSS26.0 and Amos23.0 were used for statistical analysis.Statistical methods include descriptive statistical analysis,Pearson correlation analysis,independent sample T-test,one-way ANOVA analysis,multiple linear stepwise regression analysis and constructing structural equation model.Results:1.The total score of benefit finding of patients with cervical cancer was(65.38±12.70).The average scores of each dimension were health behavior(3.12±0.90),family relationship(3.11±1.00),social relationship(3.02±0.84),personal growth(3.01±0.69),acceptance(2.99±0.92)and world outlook(2.53±0.85).The symptom burden score of cervical cancer patients was(2.91±1.79),and the symptom severity score was(2.76±1.71).The highest scores were sleep uneasiness and fatigue,which were(4.13±3.04)and(3.99±3.10),respectively.The lowest score was shortness of breath(1.89±2.26).The score of influence on daily life was(3.26±2.17).The highest score was on general activities,work or housework,which were(4.11±3.00)and(3.49±3.02),respectively,and the lowest was relationship with others.The score was(2.80±2.86).In the communication pattern of marriage with cervical cancer patients,the average scores of the three dimensions were(3.04±1.86),(2.56±1.64)and(2.37±1.54)respectively.2.The results of Pearson correlation analysis showed that the total score of benefit finding and its dimensions were significantly negatively correlated with the total score of symptom burden and its dimensions(P<0.01).The total score of benefit finding and its dimensions were significantly positively correlated with the score of constructive communication,and negatively correlated with the score of demanding withdraw communication and the score of mutual avoidance communication(P<0.01).The total score and each dimension of symptom burden were significantly negatively correlated with the score of constructive communication,and positively correlated with the demanding withdraw communication and the mutual avoidance communication.(P<0.01).3.The results of independent sample t-test and one-way ANOVA showed that there were significant differences in benefit finding scores among cervical cancer patients with different education,occupation,place of residence,working status,family income and medical expenses(P<0.01).4.Multiple linear stepwise regression analysis showed that educational level,family income,symptom burden,and the communication pattern of marriage were the predictors of perceived benefit from disease education level,family income,symptom burden and the communication pattern of marriage were predictors of benefit finding(P<0.05),which together explained 42.1% of the variance.5.The structural equation model is used to analyze the intermediary effect of husband and wife communication model,and the fitting indexes are as follows: CMIN/DF=2.204,GFI=0.946,AGFI=0.908,NFI=0.929,TLI=0.934,CFI=0.959,RFI=0.966,RMSEA=0.071.The structural equation model showed that the communication pattern of marriage played a partial mediating role in the relationship between the burden of symptoms and the benefit finding,with the mediating effect =0.169+0.134+0.085=0.388,accounting for 64.03% of the total effect.Conclusion:1.Patients with cervical cancer generally have benefit finding and are at a lower-middle level,which needs to be further improved;The symptom burden level of cervical cancer patients are at a mild level;Patients with cervical cancer mostly use the demanding withdraw communication,followed by constructive communication,and seldom use the communication of mutual avoidance.2.The benefit finding of cervical cancer patients is related to the symptom burden and the communication pattern of marriage;The main factors affecting the benefit finding of patients with cervical cancer include education level,family income,symptom burden and communication pattern of marriage.3.The communication pattern of marriage plays a partial intermediary role between symptom burden and benefit finding.The symptom burden of patients with cervical cancer can directly affect their benefit finding,and can also indirectly affect their benefit finding through the communication pattern of marriage.
