Objective:Qilu zang-fu tuina is an important branch of Qilu tuina school.Drawing on the advantages of Qilu tuina schools,Qilu tuina school’s characteristic techniques-moving with the arm, vibration with the palm, circular rubbing manipulation, pressing and kneading manipulation,thumb-pushing,circular-rubbing manipulation and other methods are comprehensively applied to the treatment of internal medicine and gynecological diseases, with remarkable clinical effect. In order to further promote and develop Qilu zang-fu tuina, this topic is on the basis of the theory of"Tong-He",Qilu zang-fu tuina is applied to treat menstrual migraine(Qi and blood stasis type),and compared with conventional tuina treatment,to observe and evaluate the clinical effect of Qilu zang-fu tuina on menstrual migraine of Qi and blood stasis type, and to explore a new idea of Qilu Zang-fu tuina in the treatment of menstrual migraine. Methods:In this study,60 patients with menstrual migraine of Qi and blood stasis type were included. The patients were divided into treatment group and control group by Visual Binning randomly, with 30 cases in each group. The treatment group was treated with Qilu zang-fu tuina, while the control group was treated with conventional tuina. The patients in both groups started treatment 14 days before menstruation, treated once the next day, stopped when menstruation came, and treated for 3 consecutive menstrual cycles. The clinical effectiveness of the two groups were evaluated with total score of headache and TCM syndrome score of menstrual migraine. Results:After 3 menstrual cycles of treatment,57 cases of clinical study were completed. There were 3 cases of exfoliation,of which 1 case in the treatment group, which failed to be treated in accordance with the plan due to the patient’s work; There were 2 cases of exfoliation in the control group,1 case affected the judgment of curative effect due to taking intervention drugs during treatment, and 1 case had poor compliance and did not continue treatment. The clinical efficacy of the patients before and after treatment was evaluated by the headache score scale and the TCM syndrome integral scale.Through the statistical analysis of the data,we can come to the conclusions:1.Primary clinical efficacy analysis:The headache score scale was used to observe the change of total headache score before and after treatment, and to compare the efficacy of headache in two groups to evaluate the primary clinical efficacy. After treatment,in the treatment group,4 cases were cured,18 cases were significantly effective,5 cases were effectual, and 2 cases were fruitless. The total effective rate was 93.1%.In the control group,1 case was cured,11 cases were significantly effective,8 cases were effectual, and 8 cases were fruitless. The total effective rate was 71.43%.After treatment, the total score of headache in both groups decreased, and there was a significant difference in both groups(P<0.01),indicating that both treatment methods can effectively improve headache symptoms. After treatment, the headache scores of the two groups were compared with each other, and statistically significant(P<0.05),indicating that the influence of improving headache symptoms in the treatment group was more significant than that in the control group. The difference in total effective rate among the two groups was statistically significant(P<0.05),indicating that the headache effect of the treatment group was better than that of the control group.2.Secondary clinical efficacy analysis:The changes of TCM syndrome integral before and after treatment were observed with the menstrual migraine TCM syndrome integral scale, and the curative effect of TCM syndrome in the two groups was compared. After treatment,in the treatment group,2 cases were cured,18 cases were significantly effective,6 cases were effectual, and 3 cases were fruitless. The total effective rate was 89.66%.In the control group,0 cases were cured,9 cases were significantly effective,9 cases were effectual, and 10 cases were fruitless. The total effective rate was 64.29%.After treatment, the score of TCM syndrome in both groups decreased, and there was a significant difference in both groups(P<0.01),indicating that both treatment methods can effectively improve the situation of TCM syndrome. After treatment, the scores of TCM syndromes in the two groups were compared with each other, and statistically significant(P<0.05),indicating that the influence of improving TCM syndromes in the treatment group was more significant than that in the control group. The difference of total effective rate among the two groups was statistically significant(P<0.05),indicating that the curative potency of TCM syndrome of the treatment group was better than control group.3.Recurrence rate analysis:The third menstrual cycle after treatment was followed up.The number of relapse was lower in the treatment group, and statistically significant(P<0.05).Conclusion:Qilu zang-fu tuina based on the theory of "Tong-He" can effectively treat menstrual migraine(Qi and blood stasis type),significantly improve headache symptoms, reduce the recurrence of menstrual migraine, make the life ameliorate of patients, provide new ideas and methods for the treatment of menstrual migraine.
