Objective:To explore whether acupoint massage combined with oral movement intervention is effective in improving sucking and swallowing dysfunction of premature infants,and to evaluate the value of this method in clinical promotion,so as to provide a more optimized treatment for complete oral feeding of premature infants.Methods:A total of 60 premature infants who met the inclusion criteria were selected from the Department of Neonatal Pediatrics of the affiliated Hospital of traditional Chinese Medicine of Southwest Medical University from June 2022 to December 2022.They were randomly divided into control group(n = 30)and intervention group(n = 30).According to the Clinical Application Guide of Neonatal Nutrition support in China,the premature infants were managed strictly according to the management method of neonatal intensive care unit.On this basis,the control group was given non-nutritive sucking(NNS),and the intervention group was given acupoint massage combined with oral movement intervention on the basis of the control group.Through the comparison of the general data of premature infants and mothers,it is concluded that the statistical difference between the two groups is comparable,and the improvement of oral motor function(tongue,mandibular movement,etc.)is evaluated by comparing the scores of Neonatal Oral Motor rating scale(NOMAS)between the two groups.The process of oral feeding was evaluated by comparing the establishment of sucking-swallowing time,gastric tube indwelling time and total enteral feeding time between the two groups.the difference of prognosis between the two groups was evaluated by comparing the time of recovery of birth weight,average daily weight growth during hospitalization and total hospitalization time between the two groups.By comparing the two groups of children with feeding intolerance(FI)to evaluate the difference of adverse reactions between the two groups,and finally achieve the purpose of evaluating the clinical efficacy.Results:The main results were as follows:(1)the general data of the two groups of premature infants: sex,mode of birth(natural delivery,caesarean section),gestational age,1 minute Apgar score after birth,birth weight,type of feeding(premature milk,breast milk,premature infant milk plus breast milk mixed feeding),days of auxiliary ventilation,mother’s age,pregnancy and delivery times,there was no significant difference(P > 0.05).(2)comparison of clinical data: NOMAS score was performed at the beginning of oral feeding,adequate oral feeding and discharge.The results showed that there was no significant difference between the intervention group and the control group in helping premature infants to establish normal sucking function at the beginning of oral feeding.The improvement of oral movement in the intervention group was better than that in the control group at other time points.The difference was statistically significant(P < 0.05).In the process of oral feeding(establishment of sucking and swallowing time,gastric tube indwelling time,total enteral feeding time),the intervention group was better than the control group(P < 0.05).The prognosis of premature infants in the intervention group(recovery time of birth weight,average daily weight increase during hospitalization and total length of stay in hospital)was significantly better than that in the control group(P < 0.05).The incidence of feeding intolerance in the intervention group was lower than that in the control group,and the difference was statistically significant(P < 0.05).Conclusion:The main results are as follows:(1)Acupoint massage combined with oral movement intervention has no significant effect on the establishment of normal sucking function of premature infants at the beginning of oral feeding,but it can correct the disorder of sucking and swallowing during the whole intervention period.to achieve the purpose of establishing normal oral motor function,to enable children to complete sucking and swallowing movements independently,and to coordinate breathing movements.(2)Acupoint massage combined with oral movement intervention showed advantages in shortening the establishment time of sucking and swallowing of premature infants,and shortened the time of indwelling gastric tube and whole intestinal feeding of premature infants.(3)Acupoint massage combined with oral exercise intervention can improve the growth of body mass,shorten the time for children to recover birth weight and the length of hospitalization,and has good family economic benefits and social value.(4)Acupoint massage combined with oral exercise intervention can reduce the incidence of feeding intolerance and promote the normal establishment of gastrointestinal function.
