There are more than80%nursing procedures could be considered painfulprocedures in NICU everyday. Some of the pains can not be dealt with efficientlydue to that premature could not express pains in right ways and nurses could notestimate pains accurately either. A plenty of people have a misunderstand thatearly pains could be regarded as such an experience that could not be recalled inearly time after birth,which makes many pains ignored in NICU. Pains could notonly influence on normal development of several systems but also lead to someneurologic diseases, psychological disorders and intelligence hypoevolutismindirectly. We could divide neonate into premature and mature according to37weeks gestational age. Because the physical signs of premature could notapproach to normal level,we always offer more nursing procedures to prematurethat make premature more sensitive to stimulation. After long-term works andsurveys in clinical, we have discovered that non-pharmacological interventionscould relieve pains with advantages of simple, economic, and effective. So we putdifferent sucking substances by oral intake, aiming to explore a right way torelieving the pains for premature so as to give some directions to clinical work.Objective:To explore the effect of non-nutritive sucking, premature formula and25% glucose sucking with applicator by oral intake on premature suffering fromvenepuncture pain, so as to provide proofs on pain management for premature inclinical work.Methods:120prematures who were hospitalized in the Neonatology Department ofThe First Hospital of Jilin University from August,2011to October,2012wereselected according to some matching conditions and were divided randomly intothree groups with the same cases. The groups included non-nutritivesucking(contrastive group), sucking premature formula with applicator by oralintake(formula group) and sucking25%glucose with applicator by oral intake(glucose group). The heart rate and the blood oxygen saturation were recordedtwo minutes before the venepuncture when the premature kept quiet. Whenpremature felt painful after venepuncture, contrastive group were offerednon-nutritive sucking, and formula group were offered sucking premature formulawith applicator by oral intake.Meanwhile,glucose group were offered sucking25%glucose with applicator by oral intake. Pains were evaluated by NeonatalInfant Pain Scale(NIPS), Premature Infant Pain Profile(PIPP), the raising of heartrate, the descending of blood oxygen saturation, seconds in countenance return tobaseline and the crying cases.SPSS17.0software was applied for statistical dataanalysis, and p<0.05was defined as a statistically significant difference.Results:(1) In terms of gender, gestational age and feeding patterns, p>0.05,so the difference is not statistically significant.(2) Results show that the two experimental groups are less than thecontrastive group in crying cases and seconds in countenance return to baseline,p<0.001, so it is statistically significant. Compared with formula group, theglucose group is much shorten and exhibited significantly in seconds incountenance return to baseline (p<0.001), but the two experimental groups are notstatistically significant in the crying cases, p>0.05.(3) The two experimental groups are more significant than the contrastivegroup in the raising of heart rate and descending of blood oxygen saturation. Thetwo experimental groups are not statistically significant in comparison, p>0.05.(4) In each group,the score of NIPS after puncture is statistically lower thanthat during puncture. Compared with contrastive group, in the two experimentalgroups, the scores of PIPP after30s, the scores of NIPS during puncture and afterpuncture are statistically lower. The glucose group is lower than the formulagroup in the scores of NIPS during puncture and after puncture(p<0.05), but thedifference of the scores of PIPP after30s is not statistically significant(p>0.05).Conclusions:(1) Compared with non-nutritive sucking, sucking premature formula or25%glucose with applicator by oral intake could reduce the crying cases during thepuncture, shorten the seconds in countenance return to baseline, reduce the scoresof PIPP after30s, the scores of NIPS during and after puncture. (2) Compared with sucking premature formula, sucking25%glucose withapplicator by oral intake could also reduce the seconds in countenance return tobaseline and the scores of NIPS during puncture and after puncture.(3) Sucking premature formula or25%glucose with applicator have thesame effect in releasing the raising of heart rate and the decreasing of bloodoxygen saturation.
