BackgroundStroke is the second leading cause of death after ischemic heart disease.62%of surviving stroke patients have varying degrees of disability.Most patients with post-stroke disability have difficulties in speech,eating,movement disorders,mood disorders,cognitive impairments,and other functional disorders.Disability assessment is of great clinical significance to predict the long-term prognosis of patients,and disability assessment is an important tool to evaluate the intervention effect of delaying and preventing the progression of disability.Different assessment tools often lead to different assessment results.Objective1.The comprehensive assessment scale for post-stroke disability was revised in Chinese to make it conform to the domestic clinical environment.2.To test the reliability and validity of the comprehensive assessment scale for post-stroke disability,explore the clinical applicability of the scale in China,and provide a more accurate and convenient assessment tool for the disability assessment of stroke patients in China.3.The Chinese comprehensive assessment scale for post-stroke disability was used to investigate the status of disability in patients with stroke and analyze the influencing factors of disability in patients with stroke.Methods1.Authorized by the author team via email,the scale was sinicized by Brislin translation and back-translation method,and cultural adjustment was conducted through expert review.A preliminary survey was conducted among 25 stroke patients by convenient sampling,and a Chinese version of the comprehensive assessment scale for post-stroke disability was formed.2.From January 2021 to May 2021,203 stroke patients meeting the study criteria were conveniently selected and investigated in the Department of Neurology,the Department of Rehabilitation Physiotherapy and the Department of Neurology of a third Class A hospital in Guangzhou and a cerebrovascular hospital in Guangzhou.The Chinese version of the comprehensive assessment scale for post-stroke disability was analyzed,and its retest reliability,inter-rater reliability,content validity and criterion-related validity were verified.3.A convenience sample of 217 stroke patients was selected from July 2021 to November 2021 to analyze the disability level of patients with different demographic characteristics and disease characteristics.The influencing factors of disability were analyzed by one-way anova,independent sample T test and multiple linear regression analysis.Results1.The comprehensive assessment scale for post-stroke disability was divided into 7 grades from low to high,with a total of 16 items,with a total score of 0 to 15.The higher the score,the more severe the disability.2.Correlation analysis of retest reliability of the Comprehensive assessment scale for post-stroke disability showed ICC=0.988;The kappa coefficient of inter-rater reliability was 0.846(95%confidence interval:0.899-0.793),P<0.001;The expert evaluation results showed that the I-CVI of each item was between 0.833-1.000,and the S-CVI calculated was 0.935.Using the modified Rankin scale as the standard,its criterion-related validity coefficient was 0.896,P<0.001.3.The disability assessment score of stroke patients was(9.83±4.45).The results of multiple linear regression analysis showed that the daily self-care ability of stroke patients and the degree of nerve damage were the influencing factors of disability level.Conclusions1.The Chinese version of the Comprehensive assessment scale for post-stroke disability has a scientific and rigorous Chinese-language process,with good reliability and validity,good clinical applicability and simple operation,which can be used as a tool for the assessment of clinical stroke disability in China.2.The disability of patients after stroke in China is at a high level,and the disability degree is serious.The self-care ability of daily life and the degree of impaired neurological function are important factors affecting the disability level of patients after stroke. |
