Objective The incidence of gastrointestinal malignancies worldwide is high,and there is a lack of effective means for early diagnosis of gastrointestinal malignancies.When the disease progresses to advanced stage,the prognosis of patients is poor.This study aims to explore the expression characteristics of TK1 and CK19 in serum and cancer tissues of patients with gastrointestinal malignancy and the relationship between the expression and clinicopathological data,so as to provide an important research basis for improving the diagnosis and treatment of gastrointestinal malignancy.Methods Selected 61 patients with gastric and colorectal cancer from January 2020 to December 2021 in Bayannaoer City Hospital as the experimental group,and the control group selected 61 healthy people from the physical examination center of our hospital.The differences in serum TK1 and CK19 levels between the two groups were determined by blotting chemiluminescence and ELISA;the correlation between serum TK1 and CK19 levels in the experimental group and their clinicopathological factors was analyzed;Changes in CK19 levels,and the relationship between serum TK1 and CK19 levels and therapeutic efficacy was analyzed;the diagnostic efficacy of TK1 and CK19 with CEA,CA199,and C724 was compared,and their diagnostic efficacy for gastrointestinal malignancies was analyzed;TK1 and CK19 The combined detection was compared with the combined detection of CEA,CA199 and CA724,and the diagnostic efficacy of the combined detection was analyzed.327 cases of gastric cancer tissue,13 cases of intestinal metaplasia tissue,31 cases of chronic gastritis,49 cases of paracancerous tissue(stomach cancer),491 cases of colon cancer tissue and 72 cases of paracancerous tissue(colon cancer)were selected from the database of Nanjing Medical University;Immunohistochemistry was used to detect the expression of TK1 and CK19 proteins in tissues;the relationship between the expression of TK1 and CK19 proteins in gastric and colon cancer tissues and clinicopathological data was analyzed;univariate and multivariate analysis and analysis of patient survival curves were used to determine the impact of prognostic factors in patients.Results The levels of SERUM TK1 and CK19 in patients with gastric and colorectal cancer were significantly higher than those in healthy people(P < 0.05).There were no significant differences in serum TK1 and CK19 levels among patients with gastric and colorectal cancer in different age,gender,tumor size and degree of differentiation(P > 0.05),but there were significant differences in serum TK1 levels among patients with different tumor stages(P < 0.05).The levels of SERUM TK1 and CK19 in patients with gastric and colorectal cancer before and after treatment were significantly lower than those before treatment(P < 0.05).Serum TK1,CK19 and three common tumor markers of gastrointestinal tract showed the highest sensitivity,minimum TK1,maximum specificity and minimum CEA in detecting GASTRIC and colorectal cancer malignant tumors.CK19 and TK1 had higher AUC than the other three tumor markers.The sensitivity,specificity and AUC of serum TK1 and CK19 combined detection were 98.4%,86.9% and0.960,respectively.The co-expression rates of TK1 and CK19 proteins in gastric and colon cancer tissues were significantly higher than those in adjacent tissues(P < 0.05),and their expressions were correlated with clinicopathological data of gastric and colon cancer patients(P < 0.05).The overall survival of patients with co-expression of TK1 and CK19 protein in stomach and colon cancer tissues was lower than that of patients with negative expression or positive expression of only TK1 or CK19 protein.The co-positive expression of TK1 and CK19 protein was an independent factor affecting the prognosis of gastric and colon cancer patients.Conclusion The combined detection of serum TK1 and CK19 can be used as an important reference index for the screening of gastrointestinal malignancies,and can be applied to the screening of gastrointestinal malignancies in healthy people and high-risk groups.It is suggested to combine imaging and other examination methods for comprehensive determination.At the same time,both of them have certain value in the evaluation of therapeutic efficacy for patients with gastrointestinal malignancy.The protein expression of TK1 and CK19 in gastric and colorectal cancer has important value in evaluating the disease progression and predicting the prognosis.
