Background:colon cancer,a common disease in clinic,whose incidence rate is second only to gastric cancer,is located in the third position of digestive tract tumor,which severely affects the health and life of patients.Currently,clinical treatment of colon cancer is mainly radical surgery,radiotherapy and chemotherapy.Although these methods can improve the symptoms of patients,they are not good in the long-term outcomes and the five-year survival rate is low.Therefore,it is of great significance to strengthen the early diagnosis and early treatment of colon cancer patients to improve the prognosis of patients.Thymidine kinase(TK)is a relatively important enzyme in human body,which catalyzes the transformation of thymidine nucleotide into monophosphate thymine and is an important remedy for pyrimidine synthesis.TK has two isoenzymes,and thymidine kinase 1(TK1)is expressed in over 95% of malignant tumors.The study shows that TK1 is ideal for colon cancer patients.it can help patients to be diagnosed early and provide basis and reference for clinical treatment,as well as predict tumor recurrence,but this conclusion remains to be verified.Objective: to investigate the expression of thymidine kinase 1 in serum of colon cancer patients and provide reference for the diagnosis and treatment of patients.Methods: in May 2014-April 2016,94 cases of newly diagnosed colon cancer in Qilu Petrochemical Center Hospital were selected as observation group.Among them,21 patients with distant metastasis were unable to undergo surgical treatment and received systemic chemotherapy;73 patients underwent radical operation.refer to patients with pathologic patients clinical data,including: gender,age,,tumor differentiation degree,,lymph node metastasis,TNM stage,Ki67,Her-2,etc;In the same period,70 patients were admitted to the hospital.Two groups of TK1 level were detected by chemical luminescence combined with spot hybridization.The electrochemical method was used to measure the level of 2 groups of cancer embryo antigen(CEA),cancer antigen 19-9(CA199)..The patients with distant metastasis in observation group were treated with conventional chemotherapy,and will complete TK1 again after treatment,to determine CEA,CA199 levels,to analyze the correlation between the serum TK1 and pathology of the patients treated by surgery.TK1,CEA and CA199 were detected at 1 months and 3months after operation.The common patients were reexamined every three months,If the TK1,CEA,or CA199 was higher than normal,The patients would be examined every month,A total of 70 patients were followed up for a median of 1.5 years.The data were processed by SPSS22.0 software Results: the levels of TK1,CEA,and CA199 were higher than those in the control group(P < 0.05).The TK1 in the patients with distant metastasis were higher than those with local recurrence.(P < 0.05).In the control group,according to diagnostic sensitivity and specificity of them.The diagnosis sensitivity and specificity of the joint diagnosis of different tumor objects were higher than that of single tumor markers.Among the 94 patients in the observation group,21 patients were with distant metastasis,all the 21 patients successfully completed the predetermined chemotherapy plan.Among the patients,there are no CR patients,11 cases were PR and the efficacy rate was 52.38%,After the treatment of PR patients,The TK1 were lower than SD+PD patients,The serum TK1 of PR patients are significantly lower than before treatment(P < 0.05).The serum TK1 level was closely related to tumor diameter,lymph node metastasis,Her-2,Ki67,and TNM staging(P < 0.05).There was no statistically significant difference of gender,age,tumor status and differentiation.(P >0.0.05).The TK1 reduced slowly,2 cases is still not reduced to normal in three months after the operation.These two patients were recurrence within one year.Post-operation follow-up,imaging lesions appeared three months even six months before,the TK1 had increased which is earlier than CEA and CA199.suggesting that TK1 can predict the recurrence or metastasis of tumors.Conclusion:TK1,which is suggested judging synthetically combined with iconography,can be used in evaluating the chemotherapy effect of colon cancer patents.TK1 is regarded as one of the predictive index of the post operative effects,relapse and metastasis,but a large number of samples are still needed further confirmation.
