Objective To investigate the relationship between the expression level of thymidine kinase 1(TK1)in serum and clinicopathological parameters in patients with non-small cell lung cancer(NSCLC),and to explore its clinical value in the diagnosis,postoperative efficacy evaluation and prognosis of patients with lung cancer.Methods The expression of serum TK1 was compared between 106 patients with NSCLC,36 patients with benign lung disease and 50 healthy subjects.The correlation between serum TK1 and clinicopathological features in NSCLC patients was analyzed.The expressions of TK1,CEA,NSE,SCC-Ag and CYFRA21-1 were detected in patients with NSCLC,the sensitivity,specificity and accuracy were compared alone or in combination.43 patients who underwent radical resection of lung cancer were monitored for perioperative tumor markers,and the significance of serum TK1 in postoperative efficacy and prognosis of lung cancer patients was analyzed.Results(1)The serum TK1 level and positive rate in the NSCLC group were significantly higher than those in the benign lung disease group and the healthy control group(P<0.05),but there was no significant difference in the serum TK1 level between the benign lung disease group and the healthy control group(P>0.05).There was no significant correlation between serum TK1 level and gender,age,smoking history and lymph node metastasis status in patients with NSCLC;but the serum TK1 was closely related to pathological type,differentiation degree,TNM stage and distant metastasis of patients.(2)When TK1,CEA,NSE,SCC-Ag and CYFRA21-1 were tested separately,the sensitivity was not high,and the five combined tests could significantly improve the sensitivity and accuracy of NSCLC.(3)In 43 patients with radical resection of lung cancer,the serum TK1 level increased 1 week postoperative,and then decreased 1 month postoperative.There was no statistical difference between the 1 week postoperative and the preoperative,but 1 month after surgery the serum TK1 significantly decreased(P<0.05).Compared with preoperative,the relapse-free survival(RFS)was significantly higher in the PR group than in the SD+PD group(71.4%vs 40%,P<0.05).The serum TK1 level(2.127 ±0.297 pM)was significantly higher in the metastatic recurrent group than in the non-metastasis recurrence group(1.362 ±0.118 pM),the difference was statistically significant(P<0.05).Conclusion The level of serum TK1 in patients with NSCLC was significantly higher than that in benign lung disease and healthy control.Serum TK1 combined with CEA,NSE,SCC-Ag and CYFRA21-1 in patients with NSCLC can significantly improve the sensitivity and accuracy of lung cancer diagnosis.The dynamic monitoring of perioperative serum TK1 levels can provide a good predictive value for postoperative efficacy and prognosis.
