| Objective:To conduct a survey on the prevalence of chronic obstructive pulmonary disease(COPD)among the residents aged 40 or older in Guizhou province.To analyze the characteristics and influencing factors among high-risk group of COPD.To provide basic data for evaluating the implementation effect of COPD prevention and control measures during the 13th Five-Year Plan period.And to provide data reference for formulating prevention and control measures of COPD in the next step.Methods:9 representative counties in Guizhou Province and 5400 residents were selected according to multistage cluster random sampling.The random residents aged 40 or older and were living in the 9 counties(unmoved more than 6 months).All the respondents were investigated by questionnaire,physical examination,and lung function test.Based on GOLD 2018,COPD was defined as a postbronchodilator FEV1:FVC<0.7,and other lung diseases were excluded at the same time.Data were collected and stored by using pad.SPSS 19.0 software was used for data statistical analysis.Qualitative data using rate and percentage in descriptive analysis.Comparison of the prevalence among different groups was analyzing by using Chi-squared test and multivariate analysis was used for the statistically significant variables in single factor analysis process.The OR and 95%CI were calculated to evaluate the risk of related factors.All the statistical tests are used two-sided test,the value is 0.05 as a standard test.Results:A total of 5413 residents aged 40 or older were surveyed in this study,and 4640of them had complete data and completed lung function tests whom were finally included in the analysis.The prevalence rate of COPD was 8.71%(95%CI:7.90%~9.52%)among residents aged 40 or older in Guizhou Province.The results showed that the prevalence of COPD in males(11.73%,95%CI:10.47%~12.99%)was higher than that in females(5.16%,95%CI:4.22%~6.10%,X~2=62.585,P<0.001).The prevalence of urban residents(7.24%)was lower than rural residents(9.93%,X~2=10.490,P=0.001).The prevalence of COPD increased with age(X~2=160.401,P<0.001).Among copd patients,mild patients(GOLD1)accounted for the largest proportion(72.03%),11.63%of patients had received previous lung function examination,and the self-awareness rate of disease was 0.99%.The results of multivariate analysis showed that the elderly(50~59 OR=2.094,95%CI:1.439~3.047;60~69 OR=4.034,95%CI:2.752~5.915;≥70 OR=6.275,95%CI:4.204~9.367),current smoking(OR=2.275,95%CI:1.804~2.869),former smoking(OR=2.410,95%CI:1.664~3.490),personal history of respiratory diseases(OR=2.268,95%CI:1.781~2.887),heating with contaminated fuel(OR=1.340,95%CI:1.043~1.721)were the risk factors of COPD.Conclusions:1.In 2020,the prevalence of COPD among residents aged 40 or older in Guizhou Province was 8.71%(95%CI:7.90%~9.52%).It is lower than the prevalence rate of COPD among the population aged 40 or older in 2014-2015(13.6%).And it is also lower than the prevalence of Guizhou Province monitoring sites in 2014(10.73%).2.The highest proportion of COPD patients were classified as with mild pulmonary function(GOLD1).3.Advanced age,personal history of respiratory diseases,smoking and heating with contaminated fuel were high risk factors of COPD in Guizhou Province. |
