Clinical Observation On The Effect Of Acupoint Massage With Meridian Flow On Postpartum Lactation Of Primipara In Natural Delivery

Objective:Under the guidance of meridian flow theory,this study compares the effect of regular time acupoint massage and timed acupoint massage in promoting postpartum lactation in primipara women,and explores the operation time to give full play to the efficacy of acupoint massage,in order to enrich meridian flow theory.The application of the theory in clinical can improve the postpartum lactation status of puerpera better and increase the exclusive breastfeeding rate of newborns.Method:In this study,convenience sampling method was used to select 113 primiparas who met the admission criteria and were randomly divided into three groups: A,B,and C.Group A received routine postpartum treatment and nursing,while Group B and Group C received acupoint massage on the basis of Group A.Group B selected routine nursing hours(9:00-11:00)and(15:00-17:00)Group C selected Chen Shi(7:00-9:00)and Wei Shi(13:00-15:00)according to the theory of Meridian Stream.Acupoint massage: Take Rugen,Danzhong,Shaoze,Taichong and Zusanli points,press and massage with thumb or forefinger for 2 minutes,2 times a day,for 5consecutive days,and 42 days after delivery.Wechat or telephone follow-up was conducted on the breastfeeding status of the three groups of puerpera every day.The milk production and breastfeeding self-efficacy on the 1st,3rd and 5th day of the intervention,the start time of stage II lactation,the lactation delay rate,and the exclusive breastfeeding rate for 42 days postpartum were evaluated.Statistical software SPSS 26.0 was used to analyze the collected data.Results:1.The general data of the three groups of research subjects before the intervention was compared,and there was no statistical significance in the overall difference between the groups(P> 0.05).2.The initiation time of stage II lactation in the three groups after intervention was(56.43± 23.08),(45.58± 20.94),(38.18± 19.71)h,respectively.The start-up time of group B and group C was shorter than that of group A(P< 0.05),and there was no significant difference between group B and group C(P> 0.05).3.The incidences of delayed initiation of stage II lactation after intervention in the three groups were 33.3%(12/36),22.2%(8/36),and8.3%(3/36),respectively.The delayed rate of lactation in group C was lower than that in group A(P < 0.05),and there was no significant difference between group A and group B,and group B and group C(P >0.05).4.The rates of exclusive breastfeeding for 42 days postpartum in the three groups were 22.2%(8/36),50.0%(18/36),and 77.8%(28/36),respectively.The rates of exclusive breastfeeding in groups B and C were higher than those in group A and higher in group C than in group B(P <0.05).5.Comparison of milk production between groups: on the first day of intervention,the milk production of group C was higher than that of group A(P< 0.05);on the third day of intervention,the milk production of group B and group C was higher than that of group A(P< 0.05);On the 5th day of intervention,the milk production in groups B and C was higher than that in group A,and group C was higher than group B(P < 0.05).Intra-group comparison: group A and group B,the amount of lactation intervened on the5 th day> the 3rd day> the 1st day(P< 0.05).In group C,the amount of lactation on the 5th and 3rd days of the intervention was higher than that on the 1st day(P< 0.05),but there was no significant difference between the5 th and the 3rd day of the intervention(P> 0.05).6.Comparison of breastfeeding self-efficacy between groups: On the first day of intervention,there was no significant difference in breastfeeding self-efficacy total score and each dimension score between groups(P> 0.05).On the 3rd day of intervention,breastfeeding self-efficacy total score and skill dimension scores in groups B and C were better than those in group A(P< 0.05);group C was better than group A in confidence dimension score(P< 0.05).On the 5th day of intervention,breastfeeding self-efficacy total score and skill dimension score,group C was better than group B and group A,group B was better than group A(P< 0.05);group C was better than group B and group A in confidence dimension score(P < 0.05).Intra-group comparison: the total score and each dimension score of group A and group B,the 5th day of intervention > the 3rd day > the 1st day(P< 0.05).The total score of breastfeeding self-efficacy and the scores of each dimension in group C were better than those on the 5th and 3rd day of intervention(P<0.05),but there was no significant difference between the 5th and 3rd day of intervention(P> 0.05).Conclusions:1.Acupoint massage can promote postpartum lactation,shorten the initiation time of lactation,reduce the delayed rate of lactation,and improve the amount of lactation,breastfeeding self-efficacy,and exclusive breastfeeding rate.2.According to the theory of Meridian Flow,acupoint massage is performed at the time of the hour and the hour of the night,and the effect is better than that of acupoint massage at the regular time.
