| Objective:This paper takes the inpatients who were discharged from a third-class traditional Chinese medicine hospital in Heilongjiang Province from 2018 to 2020 and diagnosed as"cervical disc herniation"as the research object,and analyzes the basic situation of using traditional Chinese medicine to treat patients with cervical disc herniation.To understand the basic composition,changing trend and related influencing factors of hospitalization costs,so that the hospital can optimize the proportion of Chinese and western medicine expenses on the premise of ensuring the quality of medical service and the physical and mental health of patients.Effectively controlling the increase of hospitalization expenses can reduce the disease economic burden for patients and their families as well as the whole society to a certain extent.Methods:From January 1,2018 to December 31,2020,the basic situation(sex,age,place of residence,type of insurance),disease condition(hospitalization days,disease degree,herniation levels,complications,TCM syndrome types,TCM treatment)and hospitalization expenses of patients with cervical disc herniation were screened out from the hospital HIS system and electronic cases.The data obtained were analyzed,the influencing factors of hospitalization expenses were analyzed by nonparametric rank sum test,and the multiple linear regression equation was established for multi-factor analysis.the main factors affecting the hospitalization expenses of patients with cervical disc herniation were obtained.Results:1.Results:among the 1101 cases,there were 683 female patients(62.03%)than 418 male patients(37.97%).The average age of the patients was 53.87±13.21 years old.Among the types of insurance,660 patients(59.94%)use municipal medical insurance,and 36 patients(3.27%)use urban and rural basic medical insurance and railway medical insurance.2.The results of disease analysis showed that the average hospital stay of patients with cervical disc herniation was 13.92±3.65 days,of which 701(63.67%)were 10-15 days at most.In the degree of disease,699 patients(63.49%)were more than 402 patients(36.51%).There were 362(32.88%)patients with two levels of herniation,223(20.25%)and204(18.53%)patients with protrusion of cervical intervertebral disc and degenerative changes of cervical intervertebral disc,and 652(59.22%)patients with qi stagnation and blood stasis syndrome.572(51.95%)patients with complications were more than 529patients(48.05%)without complications.Among the four treatments of acupuncture,physiotherapy,massage and fumigation in traditional Chinese medicine,577 patients(52.41%)used three kinds of therapy at the same time.3.Results of hospitalization expenses for patients:the hospitalization fee consists of medicine fee(20.93%for traditional Chinese medicine,25.08%for western medicine)treatment fee 16.53%(physiotherapy fee,fumigation fee,disposal fee)acupuncture fee8.65%,massage fee 6.39%,nursing fee 4.99%,bed fee 5.93%,examination fee 7.28%and other fees 4.72%.The average hospitalization cost was 10041.52 yuan,and the mean standard error was 96.68.the minimum hospitalization cost was 3784.07 yuan and the highest was 20280.48 yuan.4.Analysis of factors affecting hospitalization expenses:in the results of univariate analysis,gender,age,place of residence,type of insurance,days of hospitalization,complications,TCM treatment(acupuncture times,physiotherapy times,massage times,fumigation times)were statistically significant(P<0.05).In multiple linear regression analysis,the main factors affecting hospitalization expenses were hospitalization days,times of massage,times of acupuncture,age,sex and times of fumigation.The multiple regression equation is obtained as follows:Y=58.276+3.275χ1+1.855χ2+11.193χ5+0.076χ11+0.365χ13-0.075χ14Conclusion:1.All the patients in our hospital use traditional Chinese medicine to treat cervical disc herniation conservatively,most of them are female patients,and the age is mainly concentrated in the middle-aged and elderly people aged 51-65,and the young patients also account for 37.23%.Therefore,the knowledge of disease prevention should be strengthened to achieve"early prevention,early detection and early treatment".2.The hospitalization cost of traditional Chinese medicine in the treatment of cervical disc herniation is high,and the condition is easy to be repeated,which results in multiple hospitalization expenses for patients and brings greater disease economic burden to patients’families and society.therefore,we should adjust the structure of expenses and effectively reduce the days of hospitalization,control the increase of hospitalization costs,and reduce the financial burden for patients and their families.3.The expenses of western medicine and traditional Chinese medicine account for the highest proportion of hospitalization expenses,and the cost of western medicine is higher than that of traditional Chinese medicine.with the development of traditional Chinese medicine,we should give full play to the role of traditional Chinese medicine in treatment and reasonably adjust the proportion of the cost of traditional Chinese medicine and western medicine.to achieve a balance between the cost of western medicine and the cost of traditional Chinese medicine.4.Hospitalization days,massage times,acupuncture times,age,sex,fumigation times are the main factors affecting hospitalization expenses. |
