Objective:Through the retrospective analysis of blood routine related parameters of patients with sudden deafness to investigate the relationship between blood routine related parameters and sudden sensorineural hearing loss.Methods:A total of 187 patients with sudden sensorineural hearing loss admitted to Fenyang Hospital of Shanxi Province from October 2017 to October 2021 and 100 patients with healthy examined people during the same period were selected.Firstly,peripheral venous blood was collected from all patients with sudden deafness and healthy examined people on the day of admission or physical examination.Then,neutrophil count,lymphocyte count,platelet count and RDW were collected.Finally,NLR and RPR values were calculated.Next,general clinical data,such as gender,age,course of disease and so on were collected from sudden deafness patients.The differences of blood routine parameters between sudden deafness group and control group were compared.Next,pure tone audiothreshold test was performed on admission and discharge of patients in sudden hearing loss group.The patients with sudden hearing loss were divided into groups according to the detection results,and the differences of blood routine parameters in each subgroup of sudden hearing loss were compared.Then,according to the treatment effect,each group was divided into the recovery group and the non-recovery group,and the differences of blood routine parameters were compared within the group.The differences of general clinical data and related blood routine parameters between the recovered group and the non-recovered group were compared.Finally,Logistic regression was used to analyze the prognostic risk factors of sudden deafness,and ROC curve was used to judge the prognostic value of related parameters.Results:1.Compared with the control group,neutrophil-to-lymphocyte ratio(NLR)and red blood cell volume distribution width-to-platelet ratio(RPR)in the sudden hearing group were higher than those in the control group,and the differences were statistically significant(P < 0.05).There were no significant differences in neutrophil count,lymphocyte count,platelet count and red blood cell volume distribution width(RDW)between sudden deafness group and control group(P > 0.05).2.There were statistically significant differences in neutrophil count lymphocyte count,RDW,PLT,NLR and RPR among different types of sudden deafness(P < 0.05).3.Compared with the subgroups of sudden deafness,the low frequency SSNHL group had the best curative effect and the recovery rate was 86.84%.The all frequency SSNHL group had the next curative effect and the recovery rate was 75.36%,while the total frequency SSNHL group and the high frequency SSNHL group had the worst curative effect and the recovery rate was 45.10%,41.38% respectively.4.Comparison of blood routine parameters between the recovered group and the unrecovered group of sudden deafness showed significant statistically differences in NLR and RPR between the recovered group and the unrecovered group(P< 0.05),and statistically significant differences in platelet count between the low frequency SSNHL group,the all frequency SSNHL group and the total frequency SSNHL(P < 0.05).There were no significant differences in neutrophil count,lymphocyte count and RDW(P > 0.05).5.There were no significant differences in age,sex,initial hearing,tinnitus,vertigo and hearing loss between the recovered group and the non-recovered group(P > 0.05).There were significant differences in course of disease,type of hearing curve,NLR and RPR(P < 0.05).Binary Logistic regression analysis showed that the course of disease,NLR and RPR were independent risk factors affecting the prognosis of sudden deafness(P < 0.05).And the Area under the cure(AUC)of NLR and RPR were respectively 0.795 and 0.746.The sensitivity and specificity of prognosis were respectively79.1% and 73.3%,62.7% and 87.5%.Conclusion:1.Neutrophil-to-lymphocyte(NLR)and red blood cell volume distribution width-to-platelet(RPR)may be correlated with the pathogenesis and prognosis of sudden deafness.2.Platelets may be associated with the prognosis of low frequency SSNHL,all frequency SSNHL and total frequency SSNHL.3.Different types of hearing curve have different prognosis.Patients with low frequency SSNHL have the best prognosis,followed by all frequency SSNHL,While patients with total frequency SSNHL and High frequency SSNHL have poor prognosis. |
