Objective:Through the retrospective analysis of clinical data,the hematological parameters related to the severity,classification and prognosis of sudden sensorineural loss(SSNHL)were analyzed to explore the etiology and pathogenesis of SSNHL,in order to provide help for clinical treatment.Methods:Collected 104 SSNHL cases which all diagnosed in the Department of Otolaryngology Head and Neck Surgery,China-Japan Union Hospital of Jilin University from September 2018 and October 2019.And randomly selected 104healthy person who underwent a general physical examination in our hospital at the same time as the control group.Blood routine,coagulation routine,fasting blood glucose and lipid test were performed in all cases.SPSS 21.0 statistical software was used for processing and analysis,the differences of blood indexes in the sudden deafness group and the control group were compared by mann-whitney rank sum test.Spearman rank correlation analysis was used to compare the correlation between the above indicators and different types of sudden deafness.Chi-squaretes test was used to compare the correlation between blood indexes,gender,age,disease ear,type of sudden deafness and prognosis of SSNHL patients.Logistic regression analysis was used to analyze the influence of multivariate difference indexes on the therapeutic effect of SSNHL.P<0.05 was statistically significant.Results:1.Comparing the curative effect of different types of SSNHL,the results showed that the low-frequency descending type had the best effect,with the total effective rate of 82.5%,which was significantly better than the flat type and the total deafness type(c~2=15.75,P=0.000;c~2=12.25,P=0.002),while the difference among other types have no significant correlation(P>0.05).2.the difference of inflammatory indicators such as neutrophil to lymphocyte ratio(NLR),platelets to lymphocyte ratio(PLR),Coagulation index include activated partial thrombin time(APTT),fibrinogen(FIB),red blood cells deposited(HCT),mean platelet volume(MPV)and fasting plasma glucose(FBC)between study group and control group that happened statistical significance(P<0.05),However,there was no significant difference in serum lipids including triglyceride(TG),cholesterol(TC),high-density lipoprotein(HDL-C)and low-density lipoprotein(LDL-C)(P>0.05).3.there was no significant correlation between inflammatory indicators,coagulation indicators,blood lipids index,fasting blood-glucose and the classification or prognosis of SSNHL founded by spearman rank(P>0.05).Performed by Multi-factor logistic regression analysis,The curative effect of low frequency type and total deafness type is obviously different(P<0.05,OR value0.178,95%CI:0.046-0.686),There were significant differences in the therapeutic effect of SSNHL patients in the age group of 20~39 years,40~59 years and the age group of>~60 years(P<0.05,OR value 0.031(95%CI:0.006-0.146),OR value 0.205(95%CI:0.051-0.815).Conclusion:1.Different types of hearing curve have different therapeutic effects on SSNHL.Low-frequency descending type has the best prognosis,followed by high-frequency descending type,Both flat type and total deafness type have poor treatment effect.2.The inflammatory markers such as NLR,PLR,APTT,FIB,HCT,MPV,and FBC were all associated with the etiology of SSNHL.The occurrence and development of SSNHL may be related to aseptic inflammation and vascular microcirculation disorder.of the inner ear system.3.All of Inflammatory indicators,coagulation function parameters,blood sugar,blood lipid parameters,disease ear and gender have no obvious correlation with types or prognosis of SSNHL.While onset age and hearing curve are the important factors that affect the prognosis of patients with SSNHL.
