Efficacy And Safety Of Platelet Concentrate In The Treatment Of Venous Ulcers: A Systematic Review And Meta-analysis

Objective: Venous ulcers(VU)are common chronic ulcers.As a novel trauma biologic therapy,platelet concentrate(PC)has not been conclusively established in the treatment of VU.This study is proposed to analyse it using a systematic evaluation and meta-analysis.Methods: Computer searches of Pub Med,Cochrane Library,Embase,China Knowledge Network,China Biomedical Literature Database and Wanfang database were applied from the time of database creation to March 2022.Randomized controlled trials of PC for the treatment of VU were searched.Two researchers independently screened the literature,extracted data and performed literature quality assessment according to inclusion and exclusion criteria.Meta-analysis was performed using Rev Man 5.4 and stata 15.0,software specific to systematic evaluation.Results: A total of 785 patients were included in 14 published randomized controlled trials.The quantitative analysis results of various studies in this Meta-analysis were as follows: 1.Platelet concentrate improved the healing rate of venous ulcers compared with conventional treatment [RR=2.17,95%CI(1.44,3.27),p=0.0002].2.Platelet concentrate cound shorten the healing time of venous ulcers compared with conventional treatment [SMD=-2.76,95%CI(-4.38,-1.15),p=0.0008].3.Platelet concentrate improved wound reduction in venous ulcers compared to conventional treatment [SMD=0.73,95%CI(0.24,1.22),p=0.004].4.Compared with conventional treatment,platelet concentrate didn’t increase adverse reactions during treatment [RR=1.09,95%CI(0.25,4.70),p=0.91].5.Platelet concentrate reduced the recurrence of venous ulcers compared with conventional treatment [RR=0.35,95%CI(0.10,1.21),p=0.10].6.Compared with conventional treatment,platelet concentrate reduced pain during treatment [SMD=-2.49,95%CI(-6.05,1.08),p=0.17].Conclusion: PC promotes wound healing in VU compared to conventional treatment,thereby increasing the rate of ulcer healing and reducing the healing time of ulcers.In addition,PC can also reduce pain during treatment,reduce ulcer recurrence,and the incidence of adverse reactions during treatment will not increase.In conclusion,PC is reliable and safe to use,and is worthy of clinical application and promotion.
