Objective:Our aim was to evaluate the effectiveness and safety of autologous platelet-rich plasma(PRP)in healing chronic venous ulcers. The objective of the present study was also to collect evidence regarding the use of platelet-rich plasma(PRP)for the treatment of chronic venous ulcers. Methods:Through searching the following keywords from CNKI,CBM,Wan Fang Date,VIP: platelet-rich plasma(PRP),venous ulcer, chronic ulcer and chronic wound. As for PubMed,Embase and Cochrane Library,we use the PICOS(P+I+S)to search,P(patricipants): "Varicose Ulcer" [Mesh],“Ulcer,Varicose”,“Ulcers,Varicose”,“Varicose Ulcers”,“Venous Stasis Ulcers”,“Stasis Ulcer,Venous”,“Stasis Ulcers,Venous”,“Ulcer,Venous Stasis”,“Ulcers,Venous Stasis”,“Venous Stasis Ulcer”,“Venous Hypertension Ulcers”,“Hypertension Ulcer,Venous”,“Hypertension Ulcers,Venous”,“Ulcer,Venous Hypertension”,“Ulcers,Venous Hypertension”,“Venous Hypertension Ulcer”,“Venous Ulcer”,“Ulcer,Venous”,“Ulcers,Venous”,“Venous Ulcers”,“Stasis Ulcer”,“Stasis Ulcers”,“Ulcer,Stasis”,“Ulcers,Stasis”.I(intervention): "Platelet-Rich Plasma" [Mesh],“Plasma,Platelet-Rich”,“Platelet Rich Plasma”.S(study): RCT(Randomized controlled trial). The researches were update on January,2018.The methodological quality of the included studies was evaluated, and date analysis were performed using the Cochrane Collaboration software Revman 5.3.Results:After repeated screening analysis of a randomized, controlled clinical trials of paper with a total of 6 in this meta-analysis.The experimental group for the application of platelet rich plasma(PRP)in treatment group, has a total of 250 patients. The compared group was treated with routine treatment, with a total of 247 patients. Thus, there are a total of 497 patients.Statistical analysis results show that using autologous platelet rich plasma to treat the patients with chronic venous ulcer. The experimental group compared to the control group, cure rate difference is statistically significant(P < 0.00001),the experimental group is better than the control group. The contrast of marked effective rate between the two groups showed that the wound was reduced by more than 50%,and there was no secretion, and the granulation tissue had normal growth. The difference is statistically significant(P=0.01),and the experimental group is superior to the control group. The comparison of effective rate between the two groups, namely, could see the growth of granulation tissue in the wound surface, and was decreased by more than 25%.The difference is statistically significant(P=0.006),and the experimental group is superior to the control group. The comparison of inefficiency between the two groups,i.e.,could see the growth of inflammatory granulation in the wound surface, and the reduction of wound surface was less than 25%,or the wound was not healed. The difference is statistically significant(P=0.003),that is,the experimental group is lower than the control group. Conclusion:The meta-analysis results showed that application of autologous platelet rich plasma(PRP)for the treatment of chronic venous ulcer had a significantly effect, which could effectively promote the wound healing. Moreover,it's also safe and feasible, and is worth to clinical application.
