Objective: To investigate the distribution of TCM syndrome types in H-type hypertension patients with cerebral infarction.To explore the correlation between TCM syndrome types and risk factors in H-type hypertension patients complicated with cerebral infarction,to provide reference for clinical differentiation of syndromes,standardization of TCM diagnosis,early disease prevention and treatment.Methods: Collect 200 H-type hypertensive patients with cerebral infarction who meet the standards in the Department of Cardiovascular,Department of Encephalopathy and Department of Cadre Health of Qingdao Hospital of Traditional Chinese Medicine.Syndrome elements are common in blood pressure,and the "Four Diagnoses and Clinical Information Collection Form for H-type Hypertension Complicated with Cerebral Infarction" was formulated to collect diagnosis and treatment information,diagnose syndrome elements,and establish a database of patients included in the study.Among them,160 cases can be combined into syndromes Types,and The correlation of each syndrome type with gender,age,hypertension course,hypertension grade,smoking history,drinking history,TG,TC,LDL-C,HDL-C,UA,hs-CRP,Hcy,GLU and other risk factors were analyzed.Result:1.Among the 160 patients,84 were male,accounting for 52.5%;76 were female,accounting for 47.5% of the total.The age group was mainly middle-aged and elderly people which is over 55 years old,and the disease duration was over 10 years,accounting for 41.3%;the cumulative proportion of hypertension grades 2 and 3 There were significant differences in Hcy distribution among 80.6% blood pressure grades(P<0.05).2.The distribution of TCM syndrome types:51 cases(31.9%)of Kidney deficiency and blood stasis type>43 cases(26.9%)of phlegm-damp blocking collaterals type>36 cases(22.5%)of Yin deficiency and Yang hyperactivity type>30 cases(18.8%)of liver and kidney Yin and blood deficiency type.There was no significant difference in gender,age and syndrome types distribution(P>0.05).There was significant difference between Hcy level and syndrome distribution(P<0.05).3.The relationship between TCM syndrome types and related risk factors: Kidney deficiency and blood stasis were positively correlated with age,Hcy level and UA.Kidney deficiency and blood stasis were negatively correlated with hs-CRP.Phlegm dampness blocking collaterals was positively correlated with BMI,TC,TG and LDL-C.Yin deficiency and Yang hyperactivity were positively correlated with hs-CRP,and Yin deficiency and Yang hyperactivity were negatively correlated with BMI,Hcy,and UA.Liver-kidney yin-blood deficiency was negatively correlated with hypertension grade,smoking,HDL-C and Hcy.Conclusion: H-type hypertensive patients with cerebral infarction are mainly characterized by kidney deficiency and blood stasis,and phlegm-dampness blocking collaterals.Both Yin deficiency and Yang hyperactivity are seen,and the pathogenesis is complex.The older the age and the higher the levels of Hcy and UA,the more likely it is to be diagnosed with kidney deficiency and blood stasis.Actively controlling the levels of Hcy and UA can intervene in the further development of kidney deficiency and blood stasis.The higher the value of BMI,TC,TG,and LDL-C,the more likely the diagnosis will be phlegm-dampness blocking collaterals.Therefore,adjusting life style,healthy diet,lowering blood lipids and weight loss can achieve the purpose of cost-effective prevention and treatment of cardiovascular and cerebrovascular diseases.The higher the hs-CRP is,the more the syndrome type diagnosis tends to be Yin deficiency and Yang hyperactivity.The higher the BMI,Hcy,and UA are,the less the syndrome type diagnosis tends to be.Type objective index reference.The higher the HDL-C and Hcy values,the higher the level of hypertension,and the less likely the patient is to be diagnosed with liver-kidney yin-blood deficiency.
