The Morphological Changes Of Choroid In Affected And Unaffected Fellow Eyes In Acute Central Serous Chorioretinopathy

Abstract/Summary:
Purpose:To investigate the changes of choroidal thickness and choriocapillaris flow area in the macular area in eyes with acute unilateral central serous chorioretinopathy(CSC),unaffected fellow eyes and healthy controls using enhanced depth imaging optical coherence tomography(EDI-OCT)and optical coherence tomography angiography(OCTA),and to discuss its clinical significances.Methods:This study was conducted among 28 acute unilateral CSC cases and 18 age-and sexmatched healthy controls.All eyes were divided into three groups:28 eyes affected with CSC in Group A,28 unaffected fellow eyes in Group B and 18 healthy controls in Group C.All cases underwent binocular best-corrected visual acuity(BCVA),EDI-OCT and OCTA examinations.Subfoveal choroidal thickness(SFCT)and choroidal thickness(CT)at 3mm superior,inferior,nasal and temporal to the fovea were measured manually.The small to medium vessel layer(SMVL)was defined as the choroidal inner layer,and the large vessel layer(LVL)was defined as the choroidal outer layer.The mean SFCT,CT,the thickness of SMVL,the thickness of LVL,the area of foveal avascular zone(FAZ),superficial retinal vascular density(SRVD),deep retinal vascular density(DRVD)and choriocapillaris flow area among three groups were compared by matched data analysis.Results:The mean SFCT and CT at 3mm superior,inferior,nasal and temporal to the fovea were significantly thicker in affected eyes and fellow eyes than in normal control eyes(P<0.01 each),and those of affected eyes were significantly thicker than those in fellow eyes(P<0.01 each).It had no significant difference in the thickness of SMVL among these groups(P>0.05 each).The thickness of LVL at fovea and 3mm superior,inferior,nasal and temporal to the fovea were significantly thicker in affected eyes and fellow eyes than in normal control eyes(P<0.01 each),and those of affected eyes were significantly thicker than those in fellow eyes(P<0.01,P<0.01,P<0.01,P<0.01,P<0.05,respectively).With fovea as the center,if we take a radius of 1mm,the flow area of choriocapillaris was significantly decreased in CSC affected eyes and fellow eyes than in normal control eyes(P<0.01 and P<0.05,respectively),and those in affected eyes was observably less than those in fellow eyes(P<0.01).If we enlarge the radius to 3mm,the choriocapillaris flow area in CSC affected eyes was less than the fellow eyes and normal control eyes(P<0.01,P<0.01,respectively)while it did not differ significantly between fellow eyes and normal control eyes(P>0.05).Similar results were observed for the thickness of neuro epithelial layer(P<0.01,P<0.01 and P>0.05,respectively).Statistical analysis indicated that the thickness of neuro epithelial layer in fovea was correlated with the height of subretinal fluid(SRF)(r=0.724,P<0.01).Conclusions:Compared with normal healthy subjects,morphological changes of choroid not only occur in affected eyes but also in unaffected fellow eyes in acute unilateral CSC patients,mainly manifested as the increase of thickness of the choroidal outer layer and decrease of choriocapillaris flow area in fovea.As non-invasive examinations,EDI-OCT and OCTA are of great value in improving our knowledge of CSC.
