Assessment Of Choroidal Thickness Of Nonarteritic Anterior Ischaemic Optic Neuropathy By Optical Coherence Tomography

Abstract/Summary:
Objectives To observe the choroidal thickness in nonarteritic anterior ischaemic optic neuropathy(NAION)measured by enhanced depth imaging optical coherence tomography and evaluate its clinical significance.Methods From december 2014 to october 2016,30 patients diagnosed with NAION(whose contralateral eye was not involved in)were studied,in which there were 17 males and 13 females.After the examinations of visual acuity test,computer optometry test,intraocular pressure test,visual field test(30-2 mode)and FFA test,we dilated the pupil of the subjects to 8mm with 0.2% compound tropicamide eye drops(Japan,Santen)and measured the choroidal thickness with EDI-OCT.Choroidal thickness of the NAION eye and contralateral eye in patient group were evaluated and recorded after onset,a month of treatment and three months of treatment.60 individuals with normal fundus,35 males and 25 females included,were set as control.The control group also accepted the same examinations as the NAION group,and the choroidal thickness of their right eyes was measured.All the images were collected by an experienced technician and evaluated by 3 experienced ophthalmologists.The average value of the three measurements was taken as the final data.All data were analyzed by SPSS 17.0 statistical software.The measurement data were analyzed by t-test,count data were compared by x~2-test.The significance level was set as 0.05.Comparison between the NAION eye and the contralateral eye was performed by paired sample t-test.Comparison between the NAION group and the control group was made by the independent sample t-test.Difference was statistically significant when P<0.05.Results 1 Differences in age,gender and refractive status between NAION group and control group were not significant(P>0.05).2 SFCT was not significantly related to MD,but was significantly related to PSD.3 SFCT: Before or after treatment,the subfoveal choroidal thickness of NAION eye was significantly thinner than that of the control group.After treatment,the subfoveal choroidal thickness in NAION eye was significantly increased(P<0.05),whereas the subfoveal choroidal thickness of the contralateral eye was not significantly changed after treatment.The change trend of the subfoveal choroidal thickness in NAION group in three time points was different,and there was significant difference between the two eyes(Pintra-group/inter-group/interaction<0.05).4 The other sites as follows: TCT 1.5 mm,NCT 1.5 mm,SCT 1.5 mm,ICT 1.5 mm,ONCT 1.5 mm,OSCT 1.5 mm,OICT 1.5mm: Befor or after treatment,the choroidal thickness of NAION eye was significantly thinner than that of the control group(P<0.05).There was no significantly change in peripapillary sites(NCT 1.5 mm,ONCT 1.5 mm,OSCT 1.5 mm,OICT 1.5 mm)before or after treatment in NAION eye(P>0.05).There was significantly change in all sites of the contralateral eye before or after treatment(P<0.05).Comparison between NAION eye and contralateral eye almost showed no significant difference in three time points and Pinter-group>0.05.Almost all sites(except NSCT 1.5 mm)difference were significant(Pintra-group<0.05,Pinteraction<0.05).5 Among the 43 sites in our research: Before or after treatment,choroidal thickness of most sites in NAION eye and contralateral eye was significantly thinner than that of the control eye(P<0.05).Before or after one month of treatment,there was no significant difference between contralateral eye and control eye beyond 0.5 mm to the optcal disk(P>0.05).Comparison between NAION eye and contralateral eye was not significant beyond 0.5 mm to the optcal disc(P>0.05).After a month of treatment,the choroidal thickness in temple of the fovea,inferior of fovea,inferior of optic and the sites above was significantly increased.After 3 months of treatment,thoroidal thickness was not significantly different around fovea but significantly different around the optic disk.Conclusions The choroidal thickness in posterior pole of the NAION eye(onset or before treatment)was significantly lower than that in the control group.A thinner sunfoveal choroidal thickness may be a new diagnostic index of NAION.
