Objective:Atherosclerosis is the pathological basis of a variety of cardiovascular and cerebrovascular diseases,which are the first cause of death in China.New research shows that Helicobacter pylori infection is related to atherosclerosis.This study will explore the risk factors of carotid atherosclerosis in the physical examination population,and explore the relationship between Helicobacter pylori infection and carotid atherosclerosis and its risk factors,so as to provide new ideas for the prevention and treatment of atherosclerosis.Methods:601 physical examiners were selected from the Healthy Check Center of Southern Branch of the 960th Hospital of the PLA Joint Logistice Support Force from January 2018 to December 2018.The general data,blood routine indexes,biochemical indexes,14C urea breath test and carotid color Doppler ultrasound were used as observation indexes.1.According to the results of carotid color Doppler ultrasound,the subjects were divided into normal carotid artery group and sclerotic carotid artery group.The two groups were analyzed by univariate and multivariate logistic regression to explore the risk factors of carotid atherosclerosis;According to the results of 14C urea breath test,the subjects were divided into Helicobacter pylori infection negative group and Helicobacter pylori infection positive group.The incidence of carotid atherosclerosis in the two groups was compared.2.The carotid atherosclerosis group was further divided into no plaque formation group(thickening group)and plaque formation group.The two groups were analyzed by univariate and multivariate logistic regression to explore the risk factors of carotid plaque formation.3.In carotid atherosclerosis group,the differences of various indexes between Hp infection negative population and HP infection positive population were compared.4.The value of homocysteine(Hcy)in the diagnosis of atherosclerosis in Helicobacter pylori infected population was analyzed by receiver operating characteristic curve.Results:1.Uni variate analysis of influencing factors of carotid atherosclerosis showed that there were significant differences in 13 items such as age,male,history of hypertension,FPG,TC,TG,LDL,Hcy,RDW,PLT,WBC,NEU,LYMP(P<0.05),which were suspected risk factors of carotid atherosclerosis.Further binary logistic analysis showed that age,male,history of hypertension,TC,Hcy is an independent risk factor for carotid atherosclerosis.Independent risk factors except history of hypertension,the incidence of carotid atherosclerosis in HP positive group was generally higher than that in HP negative group,in which Hyperhomocysteine(Hcy>15.0 μMol/L)the incidence of carotid atherosclerosis in HP infection positive group was significantly higher than that in HP infection negative group 2.Univariate analysis of carotid plaque formation showed that there were significant differences among male,HP infection,Hcy and MCV(P<0.05),which were suspicious risk factors for carotid plaque formation.Further binary logistic analysis showed that male,HP infection,Hcy and MCV were independent risk factors for carotid atherosclerosis.3.There were significant differences in Hcy,LDL and NEU between the positive and negative groups of Helicobacter pylori infection in carotid atherosclerosis population(P<0.05).4.Among HP infected people,Hcy has medium value in the diagnosis of atherosclerosis.The AUC is 0.777,the sensitivity is 71.84%,and the specificity is 77.36%.Conclusion:1.The influencing factors of carotid atherosclerosis are diverse and complex,and there are different main risk factors in different stages.2.HP infection is an independent risk factor for carotid plaque formation.It can promote atherosclerosis by combining other risk factors of atherosclerosis.3.The increase of Hcy,NEU and LDL caused by HP infection may be the cause of the occurrence and development of carotid atherosclerosis.Serum Hcy has reference value for the diagnosis of atherosclerosis in HP infected people.
