Abstract/Summary:
ObjectiveTo investigate whether Helicobacter pylori(H.pylori)infection can increase the risk factors of Colorectal adenomatous polyp(CAP),and to analyze the relationship between Hp infection and CAP related clinical characteristics.MethodsA retrospective cohort study was used in this study.Clinical data of patients undergoing colonoscopy in medical center from March 2016 to June 2019 were collected.According to the inclusion criteria,a total of 563 examination patients were included in this study,with a male/female ratio of 368/195.All patients were tested for H.pylori and colonoscopy.H.pylori was tested using the 13C Urea Breath Test(13C-UBT)and the DOB(Delta of Baseline)value was used to display the Test results.Intestinal cleansing was performed before colonoscopy to determine the presence or absence of CAP,label the number and size distribution of CAP,and conduct biopsy for pathological typing.Patients were stratified by age and sex to observe the correlation between H.pylori infection and risk of CAP in the context of these variables.Logistic regression model was used to evaluate the prediction of H.pylori infection on the risk of CAP.ResultsA total of 563 patients were included in this study,including 207 in the control group(colonoscopy was normal),356 in the experimental group(colonoscopy was abnormal),315 in the CAP group(male/female,190/125),and41 in the Inflammatory Hyperplastic Polyp(IHP)group(male/female,20/21).The infection rate of H.pylori in CAP group was significantly higher than that in control group(119/315,37.8%;44/207,21.3%)(P<0.001).The positive rate of H.pylori in people under 50 years old was significantly higher than that of participants over 50 years old(87/250,34.8%;32/65,49.2%)(P=0.033).In addition,Logistic regression analysis showed that H.pylori infection was an independent risk factor for CAP(OR=2.679;95.0%CI:1.717-4.179)(P<0.001),and was associated with the size,distribution,and pathological grading of CAP(P<0.001),but not with the number and location of CAP(P>0.001).ConclusionsIn this study,the infection rate of H.pylori in patients with CAP was high,and the positive correlation between H.pylori infection and CAP was confirmed.The CAP colorectal adenocarcinoma axis shows a close relationship between them.The study considers that H.pylori is one of the independent factors in the risk prediction of CAP,and H.pylori infection increases the risk of CAP,thus leading to the risk of Colorectal cancer(CRC).By analyzing the causes and risk factors of H.pylori infection and CAP,it is conducive to the early eradication of H.pylori,early screening of colon polyps,effective measures to prevent and treat CAP,and the establishment of standardized and individualized prevention strategies for the clinic,thus reducing the morbidity and mortality of CRC.
