Objective:In the context of a pilot construction of a modern hospital management system,this study begins with the management of unplanned secondary surgery in the hospital,establishes a comprehensive index system for the effectiveness of the pilot construction of a modern hospital management system,evaluates the effectiveness of the pilot construction in conjunction with key case in the hospital’s pilot work,summarizes the pilot experience and makes recommendations to ensure that the pilot further plays an important role in demonstration,breakthrough and driving effect of building a modern hospital management system.Methods:This study first focuses on a single indicator and takes unplanned secondary surgery as the entry point to form an expert team to manage unplanned secondary surgery using FMEA method.The second focus is on comprehensive evaluation.The literature research method and the thematic discussion method were used to initially construct an index system for assessing the effectiveness of the pilot construction of modern hospital management system.It was modified and improved through the Delphi expert consultation method to finally determine the index system.The index system was applied to perform a longitudinal comparison of data from pilot hospitals in previous years using the comparative analysis approach,and at the same time combined with the analysis of key case to assess the effectiveness of the pilot construction of hospitalsResults:1.Relying on the background of the implementation of the pilot construction work of the modern hospital management system,a hospital medical quality management team was formed in November 2019,and the FMEA method was used to sort out the perioperative processes,analyze the potential failure modes,and draw up corresponding interventions.The results indicated that the incidence of unplanned secondary surgery in the hospital in2020 was 0.30%,which was 0.13% lower compared to 2019,with a statistically significant difference(P<0.05).2.First,the literature research method and thematic discussion method are used to develop a preliminary evaluation index system for a modern hospital management system based on the basic situation of pilot hospitals;second,the Delphi expert consultation method is used to modify and improve the initial index system.The index system is given weight via the analytic hierarchy technique.Finally,a modern hospital management system assessment index system is developed,with three first-level indexes,12 second-level indices,and 49 third-level indexes.3.The index system was used to analyze data from three dimensions of structure,procedure,and results throughout the pilot implementation phase of hospitals.The indexes with excellent results were medical quality and safety,operation-related indices,and satisfaction,while the high-weight indexes that needed to be improved were beds,efficiency,and business volume.Combining the major case study of the modern hospital management system pilot work’s implementation stage,it’s been determined that Zhabei Central Hospital in Shanghai’s Jing’an District has essentially completed the primary mission of creating and perfecting the modern hospital management system.4.Five modern hospital management system experiences are given based on the evaluation results of the pilot construction: emphasizing on the management of medical quality and safety,with the improvement of government,public,and staff satisfaction as the primary criterion for measuring the hospital’s progress;The hospital achieves the institutional cornerstone of stable development by optimizing its governance structure and leading group.;The hospital’s uniform constitution and powerful party construction provide system support for orderly development;the hospital’s initial building of the information service management platform provides technical assurance for rapid development.;The hospital achieves the mechanism guaranteeing sustainable development through key internal governance and fine discipline construction.5.Finally,six recommendations are made for modern hospital management system reform and construction: effectively balance the link between fairness and efficiency,deepen hospital structure reform;Strengthen hospital constitution research and practical application,and promote hospital management modernization and standardization.To accomplish the overall guidance of hospital operations,optimize the hospital party building mechanism and function construction.Maintain a leading position in the industry’s internal governance modernization and develop the hospital’s modern management capability;Accelerate the development of the hospital talent echelon by enhancing the overall strength of hospital disciplines in a variety of ways.To ensure the public welfare of hospitals,provide unrestricted medical supervision channels.Conclusion:On a single indicator,this study relied on the background of the pilot construction of modern hospital management system and applied the FMEA method to manage unplanned secondary surgeries,and found that the FMEA method could significantly improve the quality of medical care and continuously improve the medical process.In the system pilot construction effectiveness evaluation study,the unplanned reoperation management was used as the entry point,and the pilot construction effectiveness evaluation index system of the modern hospital management system was finally constructed.Applying the index system to longitudinally compare the relevant data of the pilot hospital in previous years and analyzing the key initiatives in the pilot hospital,it was found that the hospital had accomplished the task of pilot construction of modern hospital management system better and its pilot experience was replicable.
