Objective:To retrospectively analyze the clinical effects of single-port and three-port thoracoscopic surgery in the treatment of lung cancer. Methods:A total of 88 people who underwent complete lung cancer thoracoscopic lobectomy from January 2015 to December 2017 in Shengjing Hospital of China Medical University were selected into this research,31 cases were chosen into single-port thoracoscopic surgery group while the other 57 cases into three-port group depending on surgical methods they accepted. The duration of operation, intraoperative blood loss, number of lymph node dissection, length of postoperative extubation, length of postoperative hospitalization, postoperative pain and postoperative complications were compared between the two groups.Result:The operations were completed smoothly in both groups, and no patients were transferred to thoracotomy or died.Compared with the three-hole thoracoscopy group, the duration of surgery(192.97±58.72)min vs(167.72±42.39)min,P <0.05.Intraoperative blood loss(64.52±34.82)ml vs(55.97±20.25)ml,P > 0.05;Pain score(4.71±0.86)vs(6.74±1.32)24h after surgery,P < 0.05;Postoperative extubation duration(7.39±5.68)d vs(7.42±3.29)d,P > 0.05;the length of incision(4.36±0.57)cm vs(6.35±1.31)cm,P<0.05;Lymph node dissection group number(6.74±0.73)group vs(7.05±0.79)group,P > 0.05;Postoperative length of hospital stay(11.10±5.10)d vs(10.95±3.67)d,P > 0.05;Postoperative complications: 5 cases(2 cases of pleural effusion,2 cases of arrhythmia,1 case of pulmonary infection)vs 6 cases(2 cases of pleural effusion,2 cases of atelectasis,2 cases of arrhythmia,2 cases of pulmonary infection),P>0.05.Conclusion Single-hole thoracoscopy has no significant difference from traditional three-hole thoracoscopy in lobectomy, lymph node system dissection and other aspects,so it has equivalent surgical safety. The pain and the length of incision were less, but the operation was longer. |
