Objective The aim of this study is to construct a nurse training program of Traditional Chinese Medicine(TCM)nursing technology for Community nurses based on Tyler principle and design education experiments.The feasibility and effect of the training program were verified by referring to the evaluation model of the four levels Kirkpatrick’s model.Methods(1)Questionnaire surveys and Delphi expert consultations were used.According to the Tyler principle,a questionnaire for community nurses and community residents was drafted to clarify the needs of learners and the community,and then the framework and objectives of the training program of TCM nursing technology for Community nurses were designed.Then select 15 experts from Zhejiang Province to conduct two rounds of expert correspondence and finally determine the training program of TCM Nursing Technology for Community Nurses.(2)Design educational experiments.The participating community nurses were divided into the observation group and the control group.The observation group used the TCM nursing technology training program while the control group used the conventional training program to conduct training.Then the four levels Kirkpatrick’s model assessment model was used to evaluate the training effect.Results(1)The survey results of 200 community nurses showed that 32% of them were“excellent” in terms of nursing knowledge,43% had positive attitudes,22.5% had positive behaviors,and only 4.0% nurses had “excellent” skills.Ninety-one percent of community nurses have the training needs of TCM nursing techniques.The top six demands are scraping,cold compresses of Chinese medicine,hot and humid compresses of Chinese medicine,fumigation of Chinese medicine,auricular point pressing and cupping.89.5% of community nurses think it necessary to train,75% of nurses want to participate in training organized by Chinese medicine hospitals,the main method is semi-distribution(43%),the frequency is once a month(49%),and the total period is 80-90 hours(25%).Training lecturers were mainly clinical TCM nurses(76%)and doctors(74%),and teaching methods were operating practices(95%),theoretical lectures(75%),visits(58.5%),and case studies(56.5%).(2)Survey of Social(Community residents)needs:A survey of 310 community residents showed that 60.7% of residents preferred integrated traditional Chinese and western medicine,73.4% of residents had high recognition of TCM,and the demand for TCM nursing technology was the highest(80.7%).The demands of Chinese herbal drugs,cupping therapy,moxibustion,massage and auricular point sticking were most prominent.(3)Expert opinion: Five experts constructed the framework of the training program and proposed 12 goals of community nurses’ TCM nursing training in three areas: cognition,emotion,and motor skills.For the two rounds of Delphi expert consultation of the training program,the positive coefficients of the 15 experts were 80.00% and 93.75%,the canonical coefficients were 86% and 87%,and the Kendall coordination coefficient of the second round of the secondary and tertiary indicators was 0.428 and 0.382,and the chi-square test P value is less than 0.01.Finally,there were 5 first-level entries,18 second-level entries,and 111third-level entries for community nurses’ TCM nursing technology training programs.(4)General information and academic analysis: There were no statistically significant differences between the general information and learning status of 83 community nurses before the training(P>0.05).(5)Evaluation of training reaction level: The attendance rates of the two groups were all above 85%,the average satisfaction of the observation group was(95.05±5.32)points,and the average satisfaction of the control group was(87.00±3.41)points.There was a statistical difference between the two groups(P<0.05).(6)Comparisons between groups showed that after training,the theoretical knowledge and technical assessment scores of TCM in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).Comparisons in the groups showed that after the training,the theoretical knowledge and technical assessment scores of Chinese medicine in both groups were higher than those before training,and the difference was statistically significant(P<0.05).(7)Evaluation of training behavioral level: Comparisons between groups showed that,the self-evaluation scores and other-person evaluation scores of the TCM nursing behavior in the observation group were higher than those in the control group,and the frequency and number of items of TCM teaching after training were higher than those in the control group,and the differences were statistically significant(P<0.05).Comparisons in the groups showed that,The self-evaluation scores and other-person evaluation scores of TCM nursing behavior in both groups were higher than those before training,and the difference was statistically significant(P<0.05).(8)Evaluation of training results level: At the organizational benefit level,there was no significant difference in the number of projects and the number of times of the two groups in the TCM nursing technology(P>0.05).At the personal benefit level,the theme of post-training benefit for nurses is physical and mental experience,effect experience,training advice.Conclusions(1)Firstly,through questionnaire investigation,we found that community nurses’ knowledge and skills in TCM nursing need to be improved,and there is a strong demand for training.Community residents hold a high degree of recognition of TCM,and strong demand of TCM nursing technique.(2)Secondly,this study used expert panel discussions and the Delphi method to build a training program of TCM nursing technology for Community nurses.The program includes five parts:selection criteria,training objectives,training system,organization and management,and training evaluation.It provides specific guidance for the implementation of training.(3)Finally,the feasibility and effect of the training scheme are explored from the practical level.The training practice showed that both training programs can improve community nurses’ theoretical knowledge and skills in TCM nursing,and also improve the the self-evaluation scores and other-person evaluation scores of the TCM nursing,and at the same time increase the number of projects and the number of times of TCM nursing techniques.The observation group was superior to the control group in terms of training satisfaction,theoretical knowledge of TCM nursing,evaluation of TCM nursing techniques,the self-evaluation and other-person evaluation scores of the TCM nursing,TCM nursing teaching behavior and patient satisfaction.There are no differences between the two groups in the implementation of TCM nursing techniques.(4)The training scheme of TCM nursing technology for community nurses is scientific,practical and feasible,and worthy of promotion.
