Abstract/Summary:
Objective To apply multidisciplinary treatment(MDT)to the management of rectal cancer. And explore the effect of multidisciplinary treatment(MDT)on the stage and diagnose of rectal cancer.Method All the rectal cancer patients who had operations in The Qu Zhou people`s hospital from January 2013 to December 2016 were analyzed retrospectively. The patients were divided into MDT group and non-MDT group according to whether they were managed with MDT.The conformity rates of clinical stage to pathological stage between the two groups and postoperative effects(sphincter preservation rate, postoperative intestinal fistula rate, positive rate of circumferential resection margins and postoperative intestinal obstruction rate)were compared in order to analyze the effect of MDT on the treatment of rectal cancer. Results 266 patients were collected, the numbers of MDT group and non-MDT group were 120 and 146 respectively. The conformity rate of clinical stage to pathological stage was higher in the MDT group than in the non-MDT group on both T and N stage. And the difference was significant. There was no significant difference in the postoperative effects between the two groups. The main experimental data are as follows:1 The conformity rate of clinical stage to pathological stage on T stage was93.3%(112/120)in MDT group and 84.9%(124/146)in non-MDT group. The conformity rate between the two groups was significantly different(P<0.05).2 The conformity rate of clinical stage to pathological stage on N stage was94.2%(113/120)in MDT group and 86.3%(126/146)in non-MDT group. The conformity rate between the two groups was significantly different(P<0.05).3 The sphincter preservation rate was 79.1%(95/120)in MDT group and 80.1%(117/146)in non-MDT group. The sphincter preservation rate between the two groups was not significantly different(P=0.879).4 The postoperative intestinal fistula rate was 0.83%(1/120)in MDT group and1.37%(2/146)in non-MDT group. The postoperative intestinal fistula rate between the two groups was not significantly different(P=1.0).5 The positive rate of circumferential resection margins was 0.83%(1/120)in MDT group and 1.37%(2/146)in non-MDT group. The positive rate of circumferential resection margins between the two groups was not significantly different(P=1.0).6 The postoperative intestinal obstruction rate was 3.33%(4/120)in MDT group and 4.79%(7/146)in non-MDT group. The postoperative intestinal obstruction rate between the two groups was not significantly different(P=0.759).Conclusion MDT can be more accurate in clinical stage, give optimal treatment for the rectal cancer patients and improve the postoperative efficacy indexes. It deserves further application.
