| Canine corneal ulcer is a common ophthalmic disease,and the causes of corneal ulcer include bacterial fungal infection,chemical burn,and mechanical injury.The purpose of this study was to investigate the therapeutic effects of conjunctival flap surgery on deep corneal ulcer caused by alkali burn and mechanical injury.Method: 15 healthy experimental dogs were randomly divided into blank group,mechanical injury drug treatment group,mechanical injury conjunctival flap covering group,alkali burn drug treatment group and alkali burn conjunctival flap covering group.Dogs were subjected to basic ophthalmologic examination before the trial,and mechanical injury models and alkali burn models were constructed respectively,treated surgically and/or performed local eye spot treatment with antibiotics and autologous serum.And then,corneal cloudiness,angiogenesis,sodium fluorescein staining,and ulcer severity were detected on days 3,7,14,21,and 30 of the trial,to evaluate the effectiveness of conjunctival flap conclusion on two types of corneal ulcers(alkali burn and mechanical injury).Results:(1)Corneal turbidity was highest in the alkali burn drug treatment group throughout the treatment(P<0.05),and there was significant difference between the alkali burn drug group and the mechanical injury drug group(P<0.05).However,the difference in corneal turbidity between the mechanical injury surgery group and the alkali burn surgery group was significant only on 14 th day(P<0.05),and there was no significant difference at the rest of the time(P> 0.05).(2)On days 3 and 7,the neovascularization scores of alkali burn drug group and alkali burn surgery group were higher than that of mechanical drug combination group and mechanical surgery group(P<0.05),but the difference in corneal neovascularization between the two treatments in the same corneal ulcer model was not significant(P>0.05).On days 14,21,and 30,the mechanical drug group had the lowest degree of vascular neovascularization(P<0.05),The differences in corneal vascularization between the mechanical surgery group,alkali burn surgery group,and alkali burn drug group were not significant(P>0.05).(3)The ulcer grade of the alkali burn drug group was significantly higher than that of alkali burn surgery group on day 3(P<0.05),and the mechanical drug group had the lowest ulcer grade on day 14(P<0.05).there was a significant difference between the mechanical surgery group and the alkali burn drug group compared with the mechanical drug group(P<0.05).There was no significant difference between the four groups in ulcer grade on days 7 and 21(P>0.05).The ulcer grade scores were higher in the mechanical surgery group,the alkali burn surgery group and the alkali burn drug group than that in the mechanical drug group on day 30(P<0.05),and the alkali burn drug group had the highest ulcer grade score(P<0.05),while there was no significant difference between the mechanical surgery group and the alkali burn surgery group(P>0.05).(4)On days 3 and 7,sodium fluorescein staining scores were highest in the alkali burn drug group,but there was no significant difference in corneal fluorescence staining scores between the 4 groups(P>0.05).The mechanical drug group had the lowest corneal fluorescence staining scores on day 14(P<0.05),The other three groups did not differ significantly(P>0.05).On days 21 and 30,the highest corneal fluorescence staining scores were found in the alkali burn drug group(P<0.05),but the difference in fluorescence staining scores between the mechanical surgery group and the alkali burn surgery group was not significant(P>0.05),while the corneal fluorescence staining scores in the mechanical surgery group were significantly higher than those in the mechanical drug group(P<0.05).(5)The results of the treatment efficiency of the 4 groups tested: 2 cases were cured and 4 cases were improved in the mechanical surgery group,with a total efficiency of 100%,4 cases were cured and 2 cases were improved in the mechanical drug group,with a total efficiency of 100%;3 cases were cured and 3 cases were improved in the alkali burn surgery group,with a total efficiency of 100%,and 4 cases were improved and 2 cases were invalid in the alkali burn drug group,with a total efficiency of 66.7%.Conclusion: Corneal ulcers caused by alkali burns are poorly treated with medication alone and have a longer healing time and risk of worsening infection,whereas treatment with conjunctival flap masking combined with medication can effectively reduce the inflammatory response of corneal ulcers,reduce complications,improve corneal transparency and accelerate corneal healing.In mechanically damaged corneal ulcers,treatment with antibiotics in combination with autologous serum is better than conjunctival flap treatment for corneal clarity and neovascularization,and healing time is shorter.However,in cases where pharmacological treatment is ineffective,conjunctival flap masking is clinically recommended to treat corneal ulcers caused by mechanical injury in order to preserve the integrity of the eye,relieve symptoms,and avoid possible removal of the eye after corneal perforation. |
