Inhalation anesthesia is the most common anesthesia method in small animals.In order to achieve ideal anesthesia effect,it is usually necessary to increase the dose of a single drug,and th e analgesic effect is poor.At the same time,it also increases the risk of cardiovascular system inhibition.In bone and joint surgery,the pain is significant,the anesthesia time is long,and the anesthesia risk of animals is high.Due to the limited us e of many analgesic drugs in veterinary clinic in China,multimodal analgesia in veterinary clinic has been developing continuously.In order to reduce intraoperative and postoperative pain,better meet the requirements of animal welfare and reduce the risk of anesthesia,more attention has been paid to the application of lidocaine constant rate intravenous infusion(CRI)analgesia in recent years.By comparing and analyzing the effects of different concentrations of lidocaine CRI on isoflurane maintenance anesthesia in dogs through the pre-test,the constant rate intravenous infusion rate of lidocaine in the clinical trial was determined.On this basis,the clinical trial combined with dexmedetomidine was conducted to evaluate the effects of isoflurane main tenance anesthesia on dogs undergoing bone and joint surgery,so as to lay a foundation for clinical application.Test contents:(1)determination of anesthetic effect of different doses of lidocaine CRI: each group of lidocaine CRI was infused at the same speed,with doses of 0 mg/kg/h,1 mg/kg/h,2mg/kg/h and 3 mg/kg/h respectively.Propofol was induced,isoflurane was kept anesthetized for 1h,and pain stimulation was given to dogs every 5min.The effects of muscle relaxation,analgesia,sedation,heart rate,blood pressure,body temperature and other clinical physiological indexes of each group of experimental animals were observed and recorded.(2)Clinical trials were conducted to compare and observe the effect of lidocaine combined with dexmedetomidi ne on inhalation anesthesia during bone and joint surgery in dogs,monitor the disappearance time of eyelid reflex and plantar reflex,the recovery time of eyelid reflex,the time of extubation awakening,the time of first self lifting up and the time of first self standing,and count the dosage of propofol and isoflurane in each group,The practicability and safety of clinical application were comprehensively evaluated.Test results:(1)comprehensively evaluate the anesthetic effect of different doses of lidocaine CRI on isoflurane maintenance anesthetized dogs,and determine that the anesthetic effect of lidocaine CRI of 3 mg/kg/h in a certain range is the best.Good muscle relaxation and sedation effect can be achieved after 5 minutes of maintenance anes thesia,and the best analgesic effect can be achieved after 15 minutes.(2)The clinical comparative trial found that the combination of drugs can enter the stable anesthesia period faster than the single drug,the time is reduced by 25%,and the postoperative recovery time is delayed.(3)Compared with single drug,the dosage of induction anesthesia decreased by 30% and that of maintenance anesthesia decreased by 40%;Compared with the control group,induction anesthesia decreased by 50% and maintenance anesthesia decreased by 60%.(4)During anesthesia,the body temperature,heart rate and blood pressure showed a downward trend.Compared with other groups,there was no statistical difference(P > 0.05),which were within the range of normal physiological c hanges.The recovery period was stable,and the postoperative analgesia time was significantly prolonged(P < 0.05).Based on the above test results:(1)the comprehensive monitoring results of dexmedetomidine combined with lidocaine on the anesthetic effect of dog bone and joint surgery showed that propofol induced anesthesia was stable,tracheal sensitivity was reduced,the intubation process was more smooth,and the time to enter the stable anesthesia period was significantly reduced(P <0.05).(2)The effects of combined application on heart rate,blood pressure and body temperature of dogs had no statistical difference(P > 0.05),and the use of induced anesthesia and maintenance anesthesia decreased significantly(P < 0.05).(3)The combined application h as significant analgesic effect,reduces the degree of postoperative pain,significantly prolongs the postoperative analgesia time(P < 0.05),and is more in line with the requirements of animal welfare.
