With the wave of silver hair sweeping the world,aging for the aged has become a common challenge faced by mankind.China has a large population base and a large elderly population.The elderly population is elderly and incapacitated,which makes the old-age care problem more serious.Construction d a combination of pension service system is to meet the elderly enjoy comprehensive,continuous services,the basis of the current health care and pension service system fragmentation,lack of mutual coordination,far cannot meet the demand of the elderly services,to explore the medical raise with the old-age pension services has become a construction service system an important measure.Depth has entered the aging process of tianjin,the pension pressure,tianjin as early as 2008 began to explore the medical combination of pension services,through empirical research,tianjin in certain achievements at the same time,the medical combination service there are still difficulties,tianjin pension institutions in the national total 26,ranked 23 for the endowment of beds,do not fit with the identity of the municipality directly under the central government,further in urgent need of improvement of tianjin medical keep playing in combination with the practice of existence predicament.Foreign developed countries emphasize the need of the elderly as the center,improve the coordination and coupling between delivery services and various policies,aiming to establish a high-quality service supply system featuring multi-subject collaboration,integration of funds and interdisciplinary cooperation.This study first collected the concepts of "integrated care" and "long-term care",and summarized the advanced experience of the American PACE model,The Japanese nursing care service and the Canadian PRISMA model,so as to strengthen the understanding and practical feasibility of the combination of medical care and old-age care service.Secondly,starting from the supply and demand end,this paper analyzes the current supply situation of medical care combined with pension service in Tianjin.Confirmatory factor analysis was used to determine the service demand weight and score ranking of the elderly in order to rationally allocate service resources.Thirdly,combining with interview method,this paper analyzes the dilemma of combining medical care with practice in Tianjin and deeply analyzes its reasons.Finally,countermeasures and Suggestions are proposed for the optimization of medical care combined with elderly care services in Tianjin,including optimizing top-level design,perfecting supporting policies and regulations;Deepen cooperation,promote the effective supply of combined medical and nursing services and build a multi-level medical and nursing personnel training and support system;Improve the long-term care insurance system;Actively promoting the construction of community home-based medical care combined with elderly care service system.
