Objective:To verify the route of HIV infection among young students newly reported in Jiangsu,Hunan and Sichuan provinces in 2020,and to understand their demographic characteristics, behavioral characteristics, the history of HIV testing and its related factors. To understand the risk of transmission of HIV infection among young students who infected with HIV,and also to understand AIDS prevention and control measures and means that are more acceptable to them. Methods:A cross-sectional survey was conducted, and all young students aged between 18 to 24 who were newly reported infected with HIV in Jiangsu,Hunan and Sichuan provinces in 2020 were selected.A face-to-face survey was conducted by questionnaire and interview outline.SAS 9.4 software was used for data collation and analysis.Continuous data were expressed as mean and standard deviation(x±s),while classified data were expressed as frequency(percentage,%).Chi-square test was used for inter-group comparison and correlation analysis of rates or constituent ratios, and variate and multivariate regression analysis were performed by unconditional logistic regression model to disclose the association between various related factors and the history of HIV testing.Test level ?=0.05,bilateral test.Results:1.Verification of infection routes:A total of 416 subjects were included, about 136 from Jiangsu,133 from Hunan and 147 from Sichuan. The infection route of 12 subjects was revised from heterosexual transmission route(11 cases),unknown transmission route(1 case)to homosexual transmission route, and one case was revised from homosexual transmission to heterosexual transmission route.13 cases(4.2%)were revised, including 3 cases in Jiangsu(2.2%),2 cases in Hunan(1.5%),and 8 cases in Sichuan(5.4%),respectively.2.Demographic characteristics and behavior characteristics of young students infected with HIV in the three provinces:Among 416 subjects,409 were males(98.3%)and 7 were females(1.7%).371 persons(90.7%)had homosexual contact, the youngest age of the first homosexual sexual contact was 13 years old,the average age was 18.2±1.6 years old,the first homosexual contact partners were mainly Chinese students, accounting for 91.1%,and 44.2%of them were Chinese students. The time interval between the first homosexual contact and the diagnosis of HIV infection ranged from 0 to 8 years. Before the diagnosis, most of the respondents had homosexual contact with Chinese members(228 cases,61.5%).The majority of homosexual dating methods(86.8%)was online dating, and 18.1%insisted on using condoms when homosexual contact occurred.70 had had heterosexual contact among males, accounting for 17.1%,and all females(7 cases)had had heterosexual contact. The youngest age of the first heterosexual contact was 14 years old,the average age was 18.8±1.7 years old,the first heterosexual contact partners were Chinese, accounting for 89.6%,and 59.7%of them were Chinese students. The time interval between the onset of first heterosexual contact and the diagnosis of HIV infection ranged from 0 to 6 years. Before the diagnosis, most of the respondents had heterosexual contact with Chinese students(47 cases,61.0%).The majority of heterosexual dating methods was offline dating(53.3%),and 27.3%insisted on using condoms during heterosexual contact.Thirty-two had had intersex contact. When the first sexual contact partner were students, the multi-sexual partners were mainly Chinese students and with no foreign sexual partners. If the first sexual partners were not students, the multiple sexual partners include Chinese and foreign partners.Forty-three(10.3%)had used addictive substances, all of them male.39.9%(166 cases)had been tested for HIV before diagnosis of HIV infection.Multivariate analysis showed that among those who had had homosexual contact, those aged between 20 to 24 years old at the time of diagnosis, those who had used condoms, and those who had used addictive substance were more likely to receive HIV testing before diagnosis.47.6%(OR=1.96,P<0.01),47.4%(OR=8.80,P<0.01)and 62.8%(OR=2.26,P=0.02),respectively.3.Risk of transmission and prevention needs of young students infected with HIV:The results of in-depth interviews showed that after the diagnosis of HIV infection, the frequency of sexual behavior and willingness of young students are significantly reduced, most of them would take the initiative to use condoms when sexual behavior occurred, and they had a certain willingness to make friends with informed status.Young students had a great need to acquire knowledge related to AIDS filed, and they were eager to promote effective, diversified and targeted prevention and control strategies and measures in colleges and universities. Conclusion:The wrong-reporting rate of HIV transmission routes among young students aged 18 to 24 years old newly reported in Jiangsu Province,Hunan Province and Sichuan Province in 2020 is relatively low.Among the young students infected with HIV in such three provinces, most of them are male, and homosexual infected. The proportion of people who had sex for the first time before adulthood is relatively high, and most of the sexual partners are Chinese. The choices of multiple sexual partners before diagnosis are influenced by the first sexual partners. If the first sexual object is not students, the types of sexual partners contain more varies and show more complex.Low rates of adherence to condom using during each sex.HIV testing rate before diagnosis is neither not high.Age at diagnosis,condom-using,and using of additive substances are related factors affecting the HIV testing rate before diagnosis of young students who infected with HIV through homosexual contact.Young students infected with HIV report that they have a decrease tendency of sexual behavioral willingness and actions after infection, have a high demand for knowledge acquisition and awareness promotion, and also eager to more measures which they can take part in.Therefore, diversified prevention and treatment strategies and measures should be carried out accelerated.
