Objective:To observe the staging and overall efficacy of Qinglong tail swinging therapy combined with tonifying therapy in the treatment of lumbar disc herniation of liver and kidney deficiency type,and compare the efficacy with conventional acupuncture in the treatment of lumbar disc herniation,so as to explore the best treatment scheme and provide basis and reference for the selection of clinical treatment methods.Methods:In this clinical study,prospective randomized controlled study method was used.All the patients were treated in the Department of acupuncture and moxibustion of Dongzhimen Hospital of Beijing University of traditional Chinese medicine from September 2020 to January 2021.40 patients were planned to be included and 33 patients were actually included.They were randomly divided into experimental group and control group,16 cases in the experimental group and 17 cases in the control group.During the study,3 cases fell off,30 cases actually completed,15 cases in the experimental group and 15 cases in the control group.Shenshu(bilateral),Dachangshu(bilateral),Taixi(bilateral)and Weizhong(affected side)were selected in both groups;If the pain is mainly in the back of lower limbs,Chengshan and Zhibian should be added;The pain in the lateral part of lower limbs was mainly accompanied by Yanglingquan and Huantiao.The experimental group used Qinglong tail swinging method combined with tonifying method to acupuncture bilateral Shenshu and Dachangshu.When the patient exhaled,the needle was inserted obliquely downward.After getting the Qi,the needle was lifted to the acupoint heaven,the needle tip pointed to the disease site(the patient's pain site),and the needle handle was held in hand to maintain the needle body not advancing or retreating.The needle handle was slowly swayed left and right,and the swing angle was 45°.The needle is moved back and forth as the rudder of a boat.The needle is moved back and forth once from left to right for 9 times in total.The frequency of the needle is slow and the action is gentle,so that the acupuncture induction spreads slowly to the affected area.After the needle is moved,the needle is retained,and the rest points are treated with the method of flat reinforcing and flat reducing.The control group was treated with conventional acupuncture method,and no manipulation was performed after needling Qi.The needle was kept for 20 minutes in both groups.The subjects were treated every other day,three times a week for four weeks,a total of 12 times.The patients were followed up for 4 weeks.Evaluation index:JOA score was the main evaluation index,simple McGill pain scale and ODI disability index were the secondary evaluation indexes.JOA score and simple McGill Pain Scale were evaluated before treatment,1 week,2 weeks,3 weeks,4 weeks and 4 weeks after treatment;ODI disability index was scored before treatment,4 weeks after treatment and 4 weeks after treatment.The scores of 4 weeks after treatment were used as the criteria to judge the curative effect.The therapeutic effect was judged by comparing the scores before and after treatment.The above data were analyzed by spss25.0.Result1.JOA low back pain score:intra group comparison:before and after treatment in the experimental group,JOA score difference was statistically significant(P<0.05);The difference of JOA score in the control group before and after treatment was statistically significant(P<0.05).Comparison between groups:after 4 weeks of treatment,the JOA score of the experimental group was 26.07±2.15.The JOA score of the control group was 23.13±3.00.The difference was statistically significant(P<0.05);At the follow-up,the JOA scores of the two groups were compared,and the difference was statistically significant(P<0.05).2.Simple McGill Pain Scale score:intra group comparison:the SF-MPQ score of the experimental group was significantly different before and after treatment(P<0.05);The SF-MPQ score of the control group was significantly different before and after treatment(P<0.05).Comparison between groups:after 4 weeks of treatment,the SF-MPQ score of the experimental group was 2.03 ± 1.69.The SF-MPQ score of the control group was 5.00± 3.49,the difference was statistically significant(P<0.05).During the follow-up,the SF-MPQ scores of the two groups were compared,and the difference was statistically significant(P<0.05).3.Comparison of ODI dysfunction index:intra group comparison:the difference of ODI index in the experimental group before and after treatment was statistically significant(P<0.05);The difference of ODI index in the control group before and after treatment was statistically significant(P<0.05).Inter group comparison:after 4 weeks of treatment,the ODI index of the experimental group was 0.07± 0.04.The ODI index of the control group was 0.18± 0.12,the difference was statistically significant(P<0.05).There was significant difference in ODI dysfunction index between the two groups at follow-up(P<0.05).Conclusion1.Qinglong tail swinging therapy combined with tonifying acupuncture and conventional acupuncture can improve the pain and dysfunction of lumbar disc herniation of liver and kidney deficiency type,and improve the quality of life of patients.Both acupuncture methods are safe.2.The curative effect of Qinglong tail swinging method combined with Bufa acupuncture in the treatment of lumbar disc herniation of liver and kidney deficiency type is better than that of conventional acupuncture,and the long-term curative effect is better.
