Professor Li Peiwen's Research On The Thought Of Differentiation And Treatment Of Esophageal Cancer And The Law Of Medication

Objective:To explore Professor Li Peiwen's clinical experience in the treatment of esophageal cancer,analyze its medication rule,and provide new ideas for the treatment of esophageal cancer with traditional Chinese medicine.Methods:The prescriptions of esophageal cancer patients diagnosed and treated by Professor Li Peiwen from September 2016 to October 2020 were collected.Based on SPSS Modeler 18.0,SPSS Statistics and Microsoft Excel 2019 software,the data mining analysis was carried out on Professor Li's prescriptions for esophageal cancer.The frequency of drug use,taste and meridian tropism in the source prescriptions were counted,the association rules between drugs were determined,and new derivative prescriptions were generated to analyze Professor Li's medication rule.Through the interview with Professor Li Peiwen,the analysis results were verified and discussed,and Professor Li Peiwen's thought of differentiation and treatment of esophageal cancer and prescription medication rules were systematically summarized.Results:Among the 126 prescriptions that met the standard,218 traditional Chinese medicines were used.There are 28 drugs which are used more than or equal to 20,and the top 3 drugs are Trichosanthes kirilowii(99 times),Codonopsis pilosula(92 times)and Inula(91 times),and the drugs return to lung meridian 19.89%,stomach meridian 17.30%,spleen meridian 15.49%and liver meridian 15.01%The core drug combination "Trichosanthes kirilowii-Codonopsis pilosula-Inula" was obtained by association rule analysis,and three new prescriptions were obtained by cluster analysis,which were:Formula 1:Astragalus membranaceus,Pericarpium Citri Tangerinae,Ophiopogon japonicus,Schisandra chinensis,Angelica sinensis,Lycium barbarum,Jiao Sanxian(Jiao Hawthorn,Jiao Shenqu,Jiao Malt),Endothelium Corneum Gigeriae Galli,Fructus Citri Sarcodactylis and Hedyotis diffusa;Formula 2:Fructus Trichosanthis,Bulbus Allii Macrostemonae,Shi Jianchuan Pinellia temata,Perilla,Radix Aucklandiae,Inula,Haematite,Bamboo Ru,Scutellaria barbata,Persimmon;Formula 3:Codonopsis pilosula,Smilax glabra,Fritillaria thunbergii,Carapax Trionycis and Cornus officinalis.Conclusion:Combining the data mining results with Professor Li Peiwen's interview results,the core differentiation and treatment thought of Professor Li Peiwen's "treating esophageal cancer from Qi" is summarized.It is believed that the external causes of esophageal cancer are mainly heat,phlegm,blood stasis and poison,the internal causes are mainly lack.of healthy qi and emotional distress,and the basic pathogenesis is "qi deficiency,qi inversion and qi stagnation".In treatment,strengthening spleen and regulating qi,lowering inverse dispersion are the main methods.Professor Li believes that traditional Chinese medicine is used for treatment.It is also necessary to disperse the stagnation qi from the horizontal direction,and make good use of the whole snakegourd fruit to increase the force of widening chest and dispersing stagnation.It is believed that snakegourd fruit can alleviate and prevent the patient's chest pain and the hoarseness caused by recurrent laryngeal nerve paralysis.In addition,Codonopsis pilosula and Astragalus membranaceus are commonly used to invigorate the spleen and invigorate qi,Inula and ocher to lower the adverse effects,and dried tangerine peel,Aucklandia root and bergamot to regulate qi,and Shi Jianchuan,bamboo shavings and Hedyotis diffusa to clear away heat,detoxify and resist cancer Professor Li Peiwen attaches great importance to the prevention and treatment of tumor complications,and thinks that we should pay attention to "preventing both diseases and changes".Combined with the patient's own situation,understanding of the occurrence and development of this disease and western medicine treatment,we can foresee the possible complications of patients.Early intervention with traditional Chinese medicine can have a certain preventive effect.For the common complications of esophageal cancer,corydalis tuber,Xu Changqing,fried white peony root and toosendan fruit are commonly used for pain.Sanguisorba officinalis charcoal,bletilla striata,agrimonia pilosa,lotus root node charcoal,arborvitae,Arnebia euchroma,and radix rehmanniae are commonly used for bleeding,and self-made Mingjin prescription is commonly used for the mute caused by recurrent laryngeal nerve paralysis.In addition to the prescription,Professor.Li Peiwen also attaches great importance to the psychological counseling and life nursing of patients,in order to achieve better effect by harmonizing the body and spirit.
