ObjectiveTo study the distribution of TCM constitution types and the level of immunoglobulin E(IgE)in umbilical cord blood of healthy full-term newborns,and to analyze the correlation between them and the incidence of 42-day-old infant eczema.MethodsUsing the method of prospective cohort study,261 pregnant women who were registered in the Department of Obstetrics and Gynecology of Hospital from October 2018 to April 2020 were selected.After obtaining the informed consent of themselves and their families,umbilical cord blood was collected at birth.After enough cases were collected,the total IgE level of umbilical cord blood was determined by enzyme-linked immunosorbent assay(Elisa).The newborns who met the inclusion criteria were identified by TCM physique identification scale within 24-72 hours after birth,and followed up when they came to the hospital for physical examination again 42 days after birth.The body weight,length,feeding mode,lactation diet and eczema were investigated,and the severity of eczema was scored.Finally,the statistical analysis of the data was carried out by using SPSS 25.0 software,and the statistical difference was taken as the standard of statistical difference.Results1.Among the 261 newborns,the proportion of Gentleness type,Yang-excessiveness type and Yin-excessiveness type was 67.8%,23.4%and 8.8%,respectively.2.The incidence of eczema is different in infants with different physical types.The incidence of neonatal eczema in Yang-excessiveness type was the highest,Yin-excessiveness type was the second,and Gentleness type was the lowest.The incidence of eczema in infants with biased constitutions were higher than that in Gentleness ones,and the relative risk of infantile eczema was 1.671.3.In terms of the severity of eczema,the overall severity of infantile eczema in Yang-excessiveness type newborns was higher than that of the other two kinds of constitution(P<0.01),while the severity of Gentleness-type eczema was similar to that of Yin-excessiveness-type eczema(P>0.05).4.Family history of allergy,non-exclusive breastfeeding,eating allergenic food during lactation and biased constitution were the risk factors of 42-day-old infant eczema(P<0.05).5.The mode of delivery,family history of allergy and constitution type of traditional Chinese medicine have great influence on the total IgE level of umbilical cord blood of newborns.The total IgE level of umbilical cord blood in Yang-excessiveness group is the highest,followed by Gentleness type,and Yin-excessiveness group is the lowest.There was significant difference among the three groups(P<0.05).6.The total IgE level of umbilical cord blood in infants with eczema was higher than that in healthy infants(P<0.01).The),ROC curve showed that the total IgE level in umbilical cord blood could predict the occurrence of eczema in infants at 42 days after birth.Conclusion1.The physique of healthy newborns is mainly Gentleness type,followed by Yang-excessiveness type,and Yin-excessiveness type is the least,showing a completely different distribution characteristics from adults.2.Family history of allergy,feeding way,mother’s lactation diet and TCM constitution type are the main factors affecting the occurrence of infantile eczema,and the incidence of eczema is higher in infants with partial constitution.The symptoms of infantile eczema are the most serious in Yang-excessiveness newborns,while the severity of infantile eczema is the same as that in Gentleness and Yin-excessiveness ones.3.The mode of delivery,family history of allergy and TCM constitution type are the main factors affecting the total IgE level of umbilical cord blood.The increase of total IgE in umbilical cord blood will increase the risk of eczema and can predict the incidence of 42-day-old infant eczema.
