Preliminary Study On The Effect Of Fuxudone On The Distribution Of TCM Syndrome Elements In Children With Chronic Persistent Asthma

Objective:Through the investigation of the syndrome characteristics of children with chronic persistent bronchial asthma treated with fluticasone propionate,the changes of the distribution of syndrome elements were analyzed,and it was preliminarily clear whether the use of fluticasone propionate,had an effect on children's TCM syndromes,so as to provide a reference basis for differential diagnosis and treatment of the disease.Methods:By using the methods of epidemiological investigation and syndrome differentiation,the basic information of children with bronchial asthma and the four diagnostic information of traditional Chinese medicine were collected from December 2019 to March 2021 in the Department of Pediatrics of the first affiliated Hospital of Hunan University of traditional Chinese Medicine.the patients were divided into two groups according to whether they were treated with fluticasone propionate or not,that is,traditional Chinese medicine group and traditional Chinese medicine plus fluticasone propionate group.The data were statistically processed by EXCEL and SPSS25.0.Descriptive analysis,chi-square test and one-way ANOVA were used to analyze the syndrome factors of the two groups.Results:Disease position syndrome factor:in the traditional Chinese medicine group,the disease position syndrome factor score of chronic persistent asthma children is more than 14,including lung,surface,large intestine and stomach.The syndrome factor score of lung was the highest(43.14±15.96),followed by surface,large intestine and stomach.According to the weight ratio from large to small,the order is lung>table>large intestine>stomach(see figure 1).The syndrome elements of chronic persistent asthma in traditional Chinese medicine plus fluoxetone group were mainly related to lung,surface,spleen,kidney and skin,and the syndrome factor score of lung was the highest(43.32±15.41),followed by surface,skin,kidney and spleen.According to the weight ratio,the order is lung>table>skin>kidney>spleen.There was significant difference in syndrome elements of kidney between traditional Chinese medicine group and traditional Chinese medicine plus fluticasone propionate group,followed by large intestine,both P<0.05,the difference was statistically significant.Disease syndrome factor:in the traditional Chinese medicine group,the syndrome factor score of yin deficiency disease was the highest(20.87±8.46),and the solid diseases with syndrome factor score more than 14 were wind,phlegm,drink and fire,while deficiency disease mainly involved yin deficiency,qi deficiency,qi non-solid,essence deficiency and yang deficiency.According to the proportion of weight,the order is Yin deficiency>Yin deficiency>Wind>phlegm>Qi deficiency>Fire>Qi non-solid>essence deficiency>Yang deficiency.The solid diseases of traditional Chinese medicine plus fluticasone propionate group included drink,wind,phlegm and fire,while deficiency syndrome elements included yin deficiency,qi deficiency,qi unsolidity,yang deficiency and blood deficiency.The score of qi deficiency syndrome was the highest(27.83±11.15).According to the weight ratio from large to small,the order is qi deficiency>blood deficiency=phlegm>yang deficiency=yin deficiency>wind>drinking>qi not solid>fire.There were significant differences in syndrome elements of qi deficiency between traditional Chinese medicine group and traditional Chinese medicine plus fluticasone propionate group(P<0.001),followed by yang deficiency and blood deficiency,P<0.01,the difference was statistically significant.Syndrome element combination:the frequency of lung+surface two disease position combination was the highest in both groups.In the traditional Chinese medicine group,the frequency of the combination of two diseases was the highest,among which the frequency of wind,fire,qi deficiency and yin deficiency was the highest.Among the disease syndrome elements in the adjuvant group,the frequency of four syndrome elements and more than four syndrome elements was the highest,among which there were 3 cases of four disease syndrome elements and 10 kinds of five disease syndrome elements and above combination forms,mainly involving wind,phlegm,fluid,qi deficiency,yin deficiency,qi non-solid,yang deficiency,blood deficiency and other disease syndrome elements.Conclusion:After the application of fluticasone propionate,the common syndrome elements of chronic bronchial asthma in children include spleen,kidney and skin on the basis of lung and surface,and the scores of the three syndrome factors are significantly higher than before,and the syndrome factor classification is changed from unestablished diagnosis to first grade.On the basis of wind,phlegm,drink,yin deficiency and qi deficiency,there were blood deficiency and yang deficiency,and the main syndrome factor changed from yin deficiency to qi deficiency.Except for yin deficiency,the classification of phlegm,qi deficiency,yang deficiency and blood deficiency syndrome were all changed to the second grade,indicating that the degree of pathological changes was obviously aggravated.The combination of syndrome elements of disease has changed from the combination of two diseases to the combination of four syndrome elements and above with the highest frequency,which mainly involves deficiency of qi,deficiency of yin,deficiency of yang,deficiency of blood and other syndrome elements.Studies have shown that acetophenone can have an effect on syndrome factors in children with chronic bronchial asthma,mainly in that the pathogenesis is more complex,involving more disease location and disease syndrome elements,and obvious deficiency of qi and blood yin and yang.
