Obsessive-compulsive disorder(OCD)has a high prevalence rate and can cause serious damage to individuals and society.At present,there is still no conclusion on the pathogenesis mechanism of obsessive-compulsive disorder.No matter from the perspective of psychoanalysis or cognitive behavior,it is believed that there are many social psychological factors that will have an impact on obsessive-compulsive disorder.Among them,childhood trauma,cognitive fusion and rumination are believed to play a key role in the occurrence and development of OCD.To investigate the severity of childhood trauma,rumination,cognitive fusion and obsessive-compulsive disorder in patients with obsessive-compulsive disorder,and to explore the influence of childhood trauma on the severity of obsessive-compulsive disorder in patients with obsessive-compulsive disorder and the mediating role of rumination and cognitive fusion.130 patients with OCD and 136 healthy subjects were tested by Childhood Trauma Questionnaire,Ruminative Responses Scale,Cognitive Fusion Questionnaire and Yale-Brown Obsessive Compulsive Scale.SPSS25.0 was used for data entry,sorting and statistical analysis.Statistical methods included descriptive statistical analysis,independent sample t-test,One-way ANOVA,Pearson correlation analysis,regression analysis,mediating effect test,and the construction of structural equation model by AMOS.The results are as follows:1.The emotional abuse dimension,physical abuse dimension,emotional neglect dimension,physical neglect dimension and total score in the OCD group were significantly higher than those in the healthy group,and the RRS dimension and total score and CFQ total score were also significantly higher than those in the healthy group.2.The scores of males in sexual abuse,cognitive fusion,rumination and its’ other dimensions of OCD subjects were significantly lower than those of females;There were significant age differences in physical abuse dimension and sexual abuse dimension,and the older the group was,the higher the score was.There were significant differences in educational level in symptom rumination dimension.The lower the educational level,the higher the scores of the subjects.The scores of rumination reflection dimension and symptom rumination dimension of non-only kids were significantly higher than those of only kids.The scores of obsessive thinking and OCD of unmarried subjects were significantly lower than those of married subjects.3.Childhood trauma was significantly associated with cognitive fusion,rumination,and severity of OCD.Rumination was significantly correlated with cognitive fusion and severity of OCD.Cognitive fusion was significantly associated with the severity of OCD.4.Childhood trauma can directly affect the severity of OCD,and it can also affect the severity of OCD through two indirect pathways,namely,the single mediating effect of cognitive fusion,and the chain mediating effect of rumination and cognitive fusion,which accounted for 12.1% and 7.4% of the total effect,respectively.Based on the above results,this study drew the following conclusions: the levels of childhood trauma,rumination and cognitive fusion of OCD patients were higher than those of healthy people;Childhood trauma is not only correlated with the severity of cognitive fusion,rumination and obsessive-compulsive disorder,but also can directly affect the severity of obsessive-compulsive disorder,and can successively affect the severity of obsessivecompulsive disorder through rumination and cognitive fusion.Therefore,in the prevention and treatment of obsessive-compulsive disorder in the future,attention should be paid to the childhood traumatic experience,to create a good family atmosphere,to regulate the rumination and cognitive fusion level,and to improve the mental health status from the cognitive level. |
