Objective:The hospital anxiety and depression scale(HADS)was used to explore the symptom characteristics and prevalence of anxiety and depression in patients with obstructive sleep apnea hypopnea syndrome(OSAHS),and the correlation between the severity of anxiety and depression and the gender, age, apnea hypopnea index(AHI),body mass index(BMI),nocturnal mean pulse oxygen saturation(SpO2)and other indicators of OSAHS patients,And to explore the applicability of HADS in the comprehensive health management of OSAHS patients. Methods:Epidemiological questionnaire was used to select patients who had completed polysomnography monitoring and were diagnosed with OSAHS for the first time in the sleep monitoring center of The First Hospital of Lanzhou University from december 2020 to september 2021.More than 100 copies of HADS questionnaire were distributed. After the subjects filled in the questionnaire according to the answering specifications, the researchers checked the completed questionnaire and collected it on site, and then scored and counted the symptoms strictly according to the scoring criteria.Invalid questionnaires with non-standard answers and unreliable patients were removed, and 80 valid questionnaires were included.According to the investigation results, the symptoms and incidence of anxiety and depression in OSAHS patients were analyzed.According to the subjects’ anxiety subscale(HADS.A)score, the patients were divided into no anxiety group, mild anxiety group, moderate anxiety group and severe anxiety group. The patients were divided into non-depressive group, mild depressive group, moderate depressive group and severe depressive group according to their depression subscale(HADS.D)score.The baseline data, polysomnography(PSG)parameters,HADS score and other data of the subjects in each group were counted and the differences among the groups were compared. The correlation between the severity of anxiety and depression in OSAHS patients and their gender, age,AHI,BMI,nocturnal mean SpO2 and other indicators was analyzed.Results:1.Among OSAHS patients complicated with anxiety, the frequency of adverse emotional symptoms represented by various questions in the HADS.A questionnaire was in descending order:feeling nervous or suffering(93.21%);Feeling annoyed(90.14%);Fidgeting and wanting to move(87.47%);Unable to sit comfortably(69.46%);Feeling of panic(66.83%);Worrying about one of their internal organs(51.33 percent);Have a bad feeling(37.84 percent). In OSAHS patients with depression, the frequency of adverse mood symptoms represented by various questions in the HADS.D questionnaire from high to low is:gradually depressed mood(88.98%);Loss of interest in things that used to interest you(76.29%);Feeling unhappy(70.36%);Not as optimistic as ever(67.09 percent);Failing to see the good in things(66.21 percent);Less concerned about their appearance(63.07 percent);Loss of interest in books, television and radio(49.71 percent).2.Among the patients first diagnosed with OSAHS single disease, the prevalence rate of anxiety is 41.2%and that of depression is 30%.3.Basic data of OSAHS patients with different severity of anxiety and depression in the four groups were analyzed, including gender, age and BMI,and there was no statistical significance(P>0.05).4.There were statistically significant differences in AHI and nocturnal mean SpO2 of OSAHS patients with different anxiety and depression severity among the four groups(P<0.05).5.There was no significant correlation between gender and anxiety severity in patients with OSAHS complicated with anxiety(P=0.200>0.05).There was no significant correlation between age and anxiety severity(P=0.712>0.05).There was no significant correlation between BMI and anxiety severity(P=0.215>0.05).AHI was positively correlated with anxiety severity(P=0.039<0.05,partial regression coefficient>0).The nocturnal mean SpO2 was negatively correlated with the severity of anxiety(P=0.017<0.05,partial regression coefficient<0).6.There was no significant correlation between gender and severity of depression in OSAHS patients with depression(P=0.875>0.05).There was no significant correlation between age and depression severity(P=0.727>0.05).There was no significant correlation between BMI and depression severity(P=0.515>0.05).AHI was positively correlated with the severity of depression(P=0.010<0.05,partial regression coefficient>0),and nocturnal mean SpO2 was negatively correlated with the severity of depression(P=0.008<0.05,partial regression coefficient<0).Conclusions:1.In the population with OSAHS with anxiety, the most common anxiety like symptoms of patients are feeling nervous or painful, worrying, fidgeting, wanting to move and so on.Among OSAHS patients with depression, the most common depression like symptoms of patients are gradually depressed, losing interest in things they were interested in in in the past, feeling unhappy and so on.2.Among the patients with single disease of OSAHS for the first time, the prevalence of anxiety is 41.2%,and the prevalence of depression is 30%.3.There were no significant differences in the sex ratio, age and BMI of OSAHS patients with different anxiety and depression severity. There were significant differences in AHI and nocturnal mean SpO2 among OSAHS patients with different anxiety and depression severity.4.The gender, age and BMI of OSAHS patients with anxiety and depression had no significant correlation with the severity of anxiety and depression. The AHI of OSAHS patients with anxiety and depression is positively correlated with the severity of anxiety and depression; The nocturnal mean SpO2 of OSAHS patients with anxiety and depression is negatively correlated with the severity of anxiety and depression.
