Objective To study the gait characteristics and influencing factors of clearance obstacle in stroke patients with hemiplegic foot drop by using three-dimensional gait system,and to analyze the compensatory mechanism of clearance obstacle in stroke patients with hemiplegic foot drop from the point of view of kinematics,so as to provide reliable basis for clinical rehabilitation.Methods 30 patients with hemiplegic foot drop after stroke were selected as the observation group.According to the principle consistent with the left and right sides of hemiplegic and non-hemiplegic lower limbs,30 healthy subjects were selected as control group 1 and control group 2.The gait parameters,minimum foot clearance and its variability,balance and lower limb motor function parameters were compared between the observation group and the control group.The wobble phase toe movement trajectory was drawn,and the correlation between MTC and gait parameters,balance and lower limb motor function was analyzed.Results In the observation group,compared with the non-hemiplegic side,stride frequency,maximum hip flexion,hip extension,knee bend,ankle dorsiflexion Angle,ROM and MTC of hip,knee and ankle decreased,the difference was statistically significant(P<0.05),swing phase percentage,maximum knee extension and ankle plantar flexion Angle,MTC variability increased,the difference was statistically significant(P<0.05).Compared with control group 1,step speed,step length,step frequency,percentage of swing phase,maximum hip flexion,hip extension,knee bend,ankle dorsiflexion Angle,ROM and MTC of hip,knee and ankle decreased in observation group(P<0.05).MTC variability of the maximum knee extension and ankle plantar flexion Angle increased,and the difference was statistically significant(P<0.05).Compared with control group 2,step speed,step length,step frequency,percentage of swing phase,maximum hip extension,knee bend,ankle plantar-flexion Angle,ROM and MTC of hip,knee and ankle decreased in the observation group without hemiplegia(P<0.05),while variability of maximum knee extension and MTC increased(P<0.05).The "bimodal" vertical motion track of swinging toe disappeared in patients with foot ptosis.MTC of hemiplegic group was correlated with step speed,step length,percentage of swing phase,maximum hip extension Angle,maximum knee bend Angle,maximum ankle dorsiflexion Angle,ROM,balance function and lower limb motor function(P<0.05).Conclusion The clearance ability,balance function,lower limb motor function and walking stability were significantly decreased in stroke patients with hemiplegic foot drop.The clearance obstacle of patients with foot drop is related to the movement of hip,knee,ankle,balance function and other factors.
