Objects: To analyze the relationship between neutrophil/lymphocyte ratio(NLR)and 6-month primary patency after percutaneous transluminal angioplasty(PTA)for patients with femoropopliteal arteriosclerosis obliterans.Methods: A total of 196 patients with femoropopliteal arteriosclerosis obliterans who underwent drug-coated balloon(DCB)or uncoated balloon(UCB)PTA in our hospital were included in this study.The clinical data,pathological features,absolute numbers of neutrophils and lymphocytes before and after operation were collected.According to primary patency at 6 months,it is divided into patency group(n=128)and stenosis group(n=68).Logistic multivariate regression was used to analyze the risk factors affecting 6-month primary patency of patients with femoropopliteal arteriosclerosis obliterans.Receiver operator characteristic(ROC)curve was used to evaluate the sensitivity,specificity and cut-off point value of postoperative NLR in predicting 6-month primary patency.Results: There was no statistical difference in age,male ratio,smoking history,coexisting diseases and lesion characteristics between patency and stenosis groups(P<0.05).There was no significant difference in preoperative NLR between the two groups(2.87 vs 2.61,P=0.203),postoperative NLR in the patency group was significantly lower than that in the stenosis group(3.05 vs 3.62,P=0.007),and DCB utilization rate in the patency group was significantly higher than that in the stenosis group(50.0% vs 32.4%,P=0.018).Further comparing the difference between postoperative and preoperative NLR of different balloon types,there was no significant difference between postoperative and preoperative NLR in DCB group(3.01 vs 2.74,P=0.073),while the postoperative NLR in UCB group was significantly higher than its preoperative NLR(3.43 vs 2.81,P<0.001).Multivariate regression analysis showed that postoperative NLR was an independent predictor of 6-month primary patency in patients with femoropopliteal arteriosclerosis obliterans after PTA(odds ratio: 1.367,95% confidence interval: 1.076-1.737,P=0.010).The cut-off value of postoperative NLR for predicting 6-month primary patency is 3.20,with the sensitivity of 61.8%,and the specificity of 60.2%.Conclusion: Postoperative NLR was associated with early restenosis after PTA,Postoperative NLR?3.20 indicates a higher patency rate at 6 months.
