Objective: compare and analyze the clinical application value of Allium ureteral stent and double J ureteral stent in ureteral stenosis,and further discuss the advantages and disadvantages of each of the two treatment modalities to provide a reference basis for the treatment of ureteral stenosis in clinical practice.Methods: A retrospective analysis of 51 patients with ureteral stricture admitted to the Department of Urology of our hospital from October 2019 to May 2021 was performed.Preoperative examination combined with color ultrasound of urinary system,intravenous pyelography,CTU and other related auxiliary examinations indicated moderate or severe hydronephrosis of varying degrees in the affected kidney.By undergraduate course room the same treatment group physicians under ureter mirror retrograde in Allium ureteral stent or double J ureteral stent,record the postoperative hospitalization days,operation time and cost of treatment and related complications following surgery,the surgical success rate of comparing two treatments,preoperative postoperative renal function and the change of the degree of hydronephrosis.Results :(1)Allium ureteral stent group,a total of 23 patients were included in the analysis,and the operation success rate was 100%.The average postoperative hospital stay was 5(4,7)days,the average operation time was 90(60,90)minutes,and the average treatment cost was(71081.04±1774.243)yuan.The ureteral recanalization rate was 100% 3months after surgery,91.3% 6 months after surgery,and 73.9% 1 year after surgery.Postoperative complications can be divided into two aspects,including short-term complications and long-term complications,fever in1 case,low back pain in 1 case,and hematuria in 1 case during hospitalization.After symptomatic treatment of fluid replacement,diuretic and anti-inflammatory,the clinical symptoms disappeared before discharge.Long-term complications included stent displacement in 2 cases,stent shedding in 1 case,and stent blockage in 2 cases.Color ultrasound and renal function related indicators(creatinine,blood urea nitrogen,cystatin C)of the other patients were significantly improved after operation,clinical symptoms disappeared,and the quality of life of the patients was significantly improved.(2)Double J ureteral stent group,a total of 28 patients were included in the analysis,and the success rate of surgery was 100%.The average postoperative hospital stay was 5(4,6.75)days,the average operation time was 45.5(40,60)minutes,and the average treatment cost was(12851.25±1840.848)yuan.The ureteral recanalization rate was 61.2% 3months after surgery and 40.3% 6 months after surgery.There were 3 cases of fever,3 cases of lumbago,2 cases of hematuria and 4 cases of bladder irritation during hospitalization.Long-term complications included stent displacement in 3 cases,stent shedding in 1 case,and stent blockage in 10 cases.Color ultrasound and renal function related indexes of other patients improved after operation.(3)Both the Allium ureteral stent group and double J ureteral stent group had 100% successful rates of surgery.In terms of drainage effect,the degree of hydronephrosis and renal function related indicators(creatinine,blood urea nitrogen and cystatin C)in the Allium stent group were significantly improved after surgery,3 months after surgery and 1year after surgery.The difference was statistically significant(P <0.05).In the double J ureteral stent group,the degree of hydronephrosis and related indexes of renal function were improved after operation,3months after operation and 1 year after operation,with no statistical significance(P>0.05).In terms of overall operation time and treatment cost,the Allium stent group had longer operation time and higher treatment cost.There was no significant difference in postoperative hospital stay between the two groups.Conclusion: In the treatment of ureteral stricture,both Allium ureteral stent or double J ureteral stent can effectively relieve obstruction and improve fluid accumulation.The incidence of complications and long-term complications during hospitalization of Allium ureteral stent was lower than that of double J ureteral stent,and long-term indwelling reconnection rate was higher.Compared with Allium stent group,double ureteral catheter surgery was less difficult,shorter operation time and lower single treatment cost.In clinical practice,stent selection should be reasonable according to the patient’s condition and personal situation. |
