| Objective:To observe and analyze the correlation between age and the clinical efficacy of TCM fumigation and atomization combined with meibomian gland massage in the treatment of dry eyes with abnormal lipids. Methods:This study was a prospective,non-randomized,controlled study.A total of 61patients(122 eyes)who were diagnosed as dry eyes with abnormal lipids in our hospital during 2020.08-2021.02 and met the standards of dry eyes were collected. They were divided into three groups according to age: young group(21 cases,42 eyes),middle age group(20 cases,40 eyes)and old group(20 cases,40 eyes).Three groups of patients are treatment with TCM fumigation and atomization joint meibomian gland massage and once a week,a total of treatment three times, respectively before and after the treatment7 days gathering ocular surface disease index(OSDI)score, noninvasive break-up time(NIBUT),SchirmerⅠtest(SⅠT),tear meniscus height(TMH),lipid layer state evaluation and lack of the meibomian gland scale level. To comparing curative effect, and analyze the correlation between groups and the curative effect of the differences between age.SPSS 26.0 was used for statistical analysis.Results:(1)TCM fumigation and atomization combined with meibomian gland massage was applied to treat dry eye patients with lipid abnormity in different age groups. After treatment, there was no statistically significant improvement in TMH and meibomian gland loss in the elderly group, and there were statistically significant differences in other indicators in the elderly group and in all indicators in other groups.(2)In terms of OSDI score, the elderly group had the best effect, while the young group had the least improvement. There was no significant difference between the middle-aged group and the old group, and they were better than the young group.(3)In terms of the efficacy of NIBUT,the middle-aged group had the most obvious improvement, while the young group had the lowest total effective rate.In the pair comparison among the three groups, only the middle-aged group and the young group showed statistically significant difference in efficacy.(4)Although the lipid layer status score of the young and middle-aged patients after treatment was significantly better than that of the elderly group, there was no statistically significant difference in the efficacy level between the three groups compared with that before treatment.(5)Although the proportion of meibomian gland deficiency was improved in all three groups, the total effective rate was limited, and there was no statistically significant difference among the three groups.(6)In terms of the efficacy difference of SⅠt,although there was only a significant statistical difference between the young group and the old group,it could be seen that the efficacy of SⅠt decreased with the increase of age.(7)In terms of the efficacy of TMH,the total effective rate was the highest in the middle-aged group, followed by the young group, but there was no statistical difference between the two groups. The total effective rate of the elderly group was the lowest, and the difference was statistically significant compared with that of the young group and the middle aged group.Conclusion:(1)TCM fumigation and atomization combined with meibomian gland massage therapy is effective for dry eye patients with lipid abnormality at any age.(2)The traditional Chinese medicine used in this study is not only effective in improving lipid layer, but also has certain effect in improving tear function, and the TCM fumigation and atomization combined with meibomian gland massage therapy has a better effect in improving tear function of SⅠt and TMH in younger patients.(3)TCM fumigation and atomization combined with meibomian gland massage therapy showed better treatment effect on OSDI score and NIBUT in older patients.(4)TCM fumigation and atomization combined with meibomian gland massage therapy has significant efficacy in improving the lipid layer status of the three groups of patients and without significant difference.(5)TCM fumigation and atomization combined with meibomian gland massage therapy did not significantly improve the proportion of palpebral gland loss in the three groups. |
