ObjectiveBased on the retrospective analysis of 229 cases of lung cancer diagnosed by electronic bronchoscopy,to analysis of evaluation of bronchoscopy in diagnosis of lung cancer.MethodsWe retrospectively analyzed the data of 229 lung cancer patients who have been diagnosed by electronic bronchoscopy in the Department of Respiratory Medicine,Shandong Provincial Qianfoshan Hospital between January 2015 and December 2018.These patients were hospitalized and confirmed by pathology.Medical statistic analysis by SPSS20.0.ResultsAmong 229 cases of lung cancer,the pathological types included 75 cases of squamous cell carcinoma,68 cases of adenocarcinoma,69 cases of small cell carcinoma,13 cases of adenocarcinoma and 4 cases of other types(3 cases of untyped non-small cell carcinoma,1 case of neuroendocrine carcinoma).The percentage of squamous cell carcinoma was the largest(32.75%),followed by small cell carcinoma(30.13%),adenocarcinoma(29.69%),adenocarcinoma(5.68%)and other types(1.75%).172 males and 57 females,with a ratio of 3.02% to 1.02%,were mainly squamous cell carcinoma in males,followed by small cell carcinoma and adenocarcinoma.Adenocarcinoma accounted for 54.39% of lung cancer in women,followed by small cell carcinoma.The mean age of lung cancer was62.42±9.69 years old,the percentage of the age from 51 to 70 years old was the largest,accounting for 68.56% of the total.Squamous cell carcinoma was the most common type of lung cancer.Among 229 patients,smokers was 162 and non-smokers was 67.The incidence of squamous cell carcinoma was the highest in smokers,and adenocarcinoma was the highest in non-smokers.The appearance under the fiberbronchoscopy: there were98 cases of neoplasm type,40 cases of infiltration type,18 cases of outer suppression type,5 cases of inflammatory type,52 cases of mixed type and 16 cases of normal type.Neoplasm type insquamous cell carcinoma was was higher than other types,the types of adenocar cinoma motsly were neoplasm andnfiltration type.In the patients with lung cancer whose diseased region was clear and only one,the major locations are as follows: right upper lobe,left main bronchus,right middle lobe,right inferior lobe,left lower lobe,right main bronchus,in turn,right superior lobe,left main bronchus,right middle lobe,right inferior lobe,right main bronchus,It is suggested that the main region of lung cancer are in the upper lobe of both lungs and in the left and right main bronchi.Among 84 cases of invading multilobar lung tissue,small cell carcinoma accounted for the highest proportion.Squamous cell carcinoma,adenocarcinoma,small cell lung cancer invading left and right lung.Of the 229 patients diagnosed,202 were diagnosed by bronchoscopy.The positive rate of forceps,brush,bronchoalveolar lavage and(TBNA)was 87.57%,80.17%,40.58% and95.86%,respectively.The positive rate of any two or more combined sampling methods in forceps,brush and lavage fluid was higher than that of single sampling method.Fluorescence bronchoscopy is more sensitive than ordinary bronchoscopy in the diagnosis of lung cancer.ConclusionThe pathological type of lung cancer is closely related to the clinical features and bronchoscopy.Electronic bronchoscopy is an important method for the diagnosis of lung cancer at present.1.Squamous cell carcinoma is the most common pathological type of lung cancer under bronchoscopy.2.The incidence of male lung cancer is still higher than that of female,the male is mainly squamous cell carcinoma,and female adenocarcinoma is the main.3.Adenocarcinoma and small cell carcinoma are most common under 50 years old,and squamous cell carcinoma is more common over 50 years old.4.The patients with smoking history have a high incidence of lung cancer,the patients with smoking history are mainly squamous cell carcinoma,and the patients without smoking history are mainly adenocarcinoma.5.Lung cancer occurred mainly in the upper lobe of both lungs and left and right main bronchi,and there was no difference between left lung and right lung.6.The microscopic manifestations of lung cancer are mainly intraductal hyperplasia.The bronchoscopic manifestations of different pathological types of lung cancer are different.Squamous cell carcinoma is the main pathological type of intraductal proliferation,while the cases with infiltrative type,external pressure type and normal type are mainly adenocarcinoma.7.The positive rate of different sampling methods is different,and the positive rate of the forceps is the highest in the single sampling method,and the positive rate of the combined materials is higher than that of the single material.8.Fluorescence bronchoscopy is more sensitive than ordinary bronchoscopy in the diagnosis of lung cancer.
