A Study On Acupoint Selection For Post-stroke Urinary Incontinence Treated By Acupuncture And Moxibustion Based On Data Mining Technology

Objective Using data mining technology to analyze the literature of acupuncture therapy for post-stroke urinary incontinence,and to probe into the rules of acupoint selection on acupuncture treatment of this disease.Methods 1.Data collection:All clinical literatures on acupuncture treatment of post-stroke urinary incontinence from the establishment of the databases to December 3 1,2021 collected from CNKI,SinoMed,VIP,WF,PubMed,Web of Science,Cochrane Library and Embase databases by computer retrieval.2.Data preprocessing:Duplicate files were deleted.The rest of the literatures were filtered based on inclusion and exclusion criteria.Finally,a total of 176 articles were included.3.Establishing a database:The included literatures’ data were input into the Excel table according to the name of the literature,author,literature resources,clinical design,acupuncture-moxibustion therapy prescription,therapeutic effect,then a prescription database for acupuncture treatment of post-stroke urinary incontinence was built.At the same time,the names of acupoints in the database were normalized.4.Data mining:We used Microsoft Excel 2007,IBM SPSS Modeler 18.1 and IBM SPSS Statistics 25.0 to perform frequency analysis,association analysis and cluster analysis to obtain the rules of acupoints,meridians,parts,specific acupoint frequency,syndrome differentiation,association and clustering of acupoints,then used CytoScape 3.8.0 to display the complex network of associations between acupoints.Results 1.A total of 176 acupuncture prescriptions were included in the database for acupuncture treatment of post-stroke urinary incontinence.2.Frequency analysis showed that 115 acupoints were involved in acupuncture treatment of post-stroke urinary incontinence.and the total frequency of usage of acupoints was 1165 times.The top 20 acupoints in frequency are in order as follows:Guanyuan,Zhongji,Qihai,Sanyinjiao,Baihui,Shenshu,Ciliao,Sishencong,Pangguangshu,Zhongliao,Xialiao,Shangliao,Zusanli,Shenque,Huiyang,Shuidao,Yinlingquan,Taixi,Foot Motor Sensory Area and Qugu.Frequency analysis also showed that 33 acupoints were involved in electroacupuncture treatment of post-stroke urinary incontinence,and the total frequency of usage of acupoints was 216 times.The top 10 acupoints in frequency are in order as follows:Shenshu,Ciliao,Huiyang,Zhongji,Zhongliao,Sishencong,Xialiao,Guanyuan,Shangliao,Baihui and Sanyinjiao.Frequency analysis also showed that 33 acupoints were involved in moxibustion treatment of post-stroke urinary incontinence,and the total frequency of usage of acupoints was 307 times.The top 10 acupoints in frequency are in order as follows:Guanyuan,Qihai,Zhongji,Shenque,Sanyinjiao,Shenshu,Baihui,Ciliao,Zhongliao,Pangguangshu,Shangliao,Xialiao and Zusanli.In the analysis of meridian frequency,the results suggested Conception Vessel,Bladder Meridian,Governor Vessel and Spleen Meridian ranked top four.According to the frequency,the order of acupoints position is:thorax and abdomen,back-lumbar-sacroiliac region,lower limbs,head-face-neck and upper limbs.The most-used specific points are crossing points and the least-used are clef points.Among the included literatures,only 24 articles identified TCM syndrome types and used treatment based on syndrome differentiation.The deficiency of kidney Yang,deficiency of spleen and kidney Yang,deficiency of kidney qi,dampness-heat of liver channel and deficiency of lung qi are common TCM syndromes.3.In the clustering analysis,acupoints were divided into seven types of clusters,which were as follows:the first type:Xialiao,Shangliao,Zhongliao,Ciliao,Foot Motor Sensory Area;the second type:Sishencong,Huiyang;the third type:Shuidao,Qugu;the fourth type:Neiguan,Shuigou,Baihui;the fifth type:Guanyuan,Qihai,Zhongji,Shenque;the sixth type:Shenshu,Pangguangshu;the seventh type:Sanyinjiao,Yinlingquan,Taixi,Zusanli and Mingmen.4.In the association analysis,the pairing of acupoints with support>20%and confidence>80%were:Qihai and Guanyuan,Sanyinjiao and Guanyuan,Qihai and Zhongji,and Guanyuan,Sanyinjiao and Zhongji,and Guanyuan,Baihui and Sanyinjiao,and Guanyuan,Qihai and Sanyinjiao,and Guanyuan,Baihui and Zhongji,and Guanyuan.Conclusions 1.The clinical selection of acupuncture and moxibustion for post-stroke urinary incontinence is mainly based on the local selection and adjacent selection of abdominal and lumbosacral acupoints,combined with scalp acupoints or scalp stimulation area,and acupoints of limbs based on syndrome differentiation.The mainly-used specific points are crossing points.The mainly used specific acupoints are confluent acupoints.2.Guanyuan,Zhongji,Qihai,Sanyinjiao and Baihui constitute a core acupuncture prescription for the treatment of post-stroke urinary incontinence,and Guanyuan is the core acupoint.3.The selected acupoints are mainly distributed in Conception Vessel,Bladder Meridian,Governor Vessel and Spleen Meridian.4.The compatibility of acupoints follows the principle of local point association and upper-lower point association.
