Retrospective Study On Clinical Syndrome And Treatment Characteristics Of Warm Lung-heat Disease（Community-Acquired Pneumonia）

Objective: To analyze the clinical characteristics of TCM of wind-warm lung-heat disease（community-acquired pneumonia with positive respiratory virus）through retrospective study of collected clinical cases.To compare the difference between the treatment of wind-warm lung-heat disease with integrated traditional Chinese and western medicine and the conventional treatment of wind-warm lung-heat disease with western medicine.Methods:97 cases of community-acquired pneumonia（CAP）were admitted to Huayuanshan Pulmonary Disease Department of Hubei Provincial Hospital of Traditional Chinese Medicine from October 2019 to December 2021,and the pathogenic spectrum of97 cases was analyzed.Seventy-four cases of CAP with positive respiratory virus were counted,including etiology,hospitalization time,age,sex,BMI,symptoms of traditional Chinese medicine,combined basic diseases,distribution of imaging,distribution of TCM syndrome types,body temperature,hospitalization time and therapeutic effect of different syndrome types.Seventy-four cases of CAP with positive respiratory virus were divided into the treatment group of integrated traditional Chinese and western medicine 6（IL6cases）and the treatment group of western medicine（28 cases）.Using IBM.SPSS.Statistics.25 software system,the curative effect,antipyretic time,hospitalization time,white blood cell count（WBC）,neutrophil percentage（GRAN%）,Neutrophil to lymphocyte ratio（NLR）,C-reactive protein（CRP-H）,Procalcitonin（PCT）and interleukin 6（IL6）were compared between the two groups.Results: 1.97 cases of CAP with positive pathogens: 15 cases were bacterial positive,22 cases were atypical pathogens,13 cases were fungal infection,and 74 cases were virus positive.Among the 74 CAP cases with positive respiratory viruses,there were 17 cases of virus-virus mixed infection,10 cases of virus-atypical pathogen mixed infection,4 cases of virus-bacteria mixed infection and 3 cases of virus-fungi mixed infection.2.CAP virus infection occurs in all seasons,mainly in winter;The incidence is more common in people over 60 years old;Clinical manifestations are mainly respiratory system,which is easy to affect digestive system.Basic diseases are common in cardiovascular and cerebrovascular diseases,metabolic and nutritional diseases and respiratory diseases.3.The TCM syndromes of community-acquired pneumonia with positive respiratory virus are mainly divided into four types,namely,the syndrome of evil attacking lung health（77.46%）,the syndrome of phlegm-heat stagnating lung（20.27%）,the syndrome of phlegm obstructing lung（4.05%）,and the syndrome of positive deficiency and evil love（1.35%）.The average body temperature of the syndrome of attacking the lung is the highest,followed by the syndrome of phlegm-heat stagnation of the lung;Deficiency syndrome has a long course of disease,followed by phlegm-heat stagnation of lung syndrome;4.Comparison of clinical curative effect.Under the same curative effect,the antipyretic time and hospitalization days in the treatment group of integrated traditional Chinese and western medicine are shorter than those in the treatment group of western medicine.Conclusion: CAP with positive respiratory virus often occurs in winter,and it is more common in people with age ≥60 and poor self-immunity or respiratory immunity.There was no significant difference in clinical symptoms between CAP and CAP.According to clinical symptoms,it can be divided into four syndrome types,namely,syndrome of evil attacking lung-health,syndrome of phlegm-heat stagnating lung,syndrome of phlegm obstructing lung,syndrome of positive deficiency and evil love,and syndrome of evil attacking lung-health is more common;There are obvious differences in body temperature and hospitalization time among different syndrome types;According to the syndrome of evil attacking lung-health,phlegm-heat stagnating lung,phlegm obstructing lung,and positive deficiency and evil-loving syndrome,the treatment principles of clearing exterior and detoxifying,clearing heat and eliminating phlegm,eliminating dampness and eliminating phlegm,strengthening body resistance and eliminating evil,etc.were adopted respectively.Combined with conventional western medicine treatment,the patients’ fever-relieving time was faster and the hospitalization time was shorter.The above results have certain reference value for CAP with positive respiratory virus.
