| [Objective]By observing the improvement of clinical symptoms,pregnancy during and within six months after treatment in patients with tubal obstructive infertility of qi stagnation and blood stasis type treated with Qingluo Enema Decoction combined with magnetic heat and microwave therapy,and analyzing the changes of IL-6,TNF-α,MA,R value and blood viscosity before and after treatment in patients,we evaluated the clinical efficacy of this treatment method on this disease and explored its mechanism of action to provide a basis for this treatment protocol in clinical To evaluate the clinical efficacy of this treatment method on this disease and to discuss its mechanism of action,so as to provide a basis for the further promotion and application of this treatment protocol in the clinic.[Methods]A total of 47 patients with tubal obstructive infertility of Qi stagnation and blood stasis type who were treated in Litongde Hospital of Zhejiang Province from August 2020 to August 2021 were enrolled.And patients’ peripheral blood was collected before and after treatment to detect relevant indicators,TCM symptom scores and local sign scores.Observation:① Changes of cytokines(IL6,TNF-α),thromboelastography(R value,MA)and blood viscosity before and after treatment,to explore the reference value of IL-6 and TNF-α indexes for these patients;② Before and after treatment Changes of TCM symptom score and local sign score;③Pregnancy during treatment and half a year after treatment to evaluate the clinical efficacy of Qingluo Enema Decoction combined with magnetic heat and microwave therapy.[Results]1.After treatment,the concentrations of IL-6 and TNF-α in patients were significantly lower than those before treatment,and the differences were statistically significant(P<0.05).2.After treatment,the whole blood low-shear reduced blood viscosity of patients decreased compared with before treatment,and the difference was statistically significant(P<0.05).The whole blood high shear reduced blood viscosity before treatment was slightly lower than that before treatment,but the difference was not statistically significant(P>0.05).3.After the treatment,the MA level of the patients decreased and the R value increased compared with that before the treatment,and the differences were statistically significant(P<0.05).4.After treatment,the total score of TCM symptoms,the total score of each symptom,and the total score of local signs were significantly lower than those before treatment,and the difference was statistically significant(P<0.05).Evaluation of the efficacy of TCM symptoms:3 cases were cured(6.82%),11 cases were markedly effective(25.00%),26 cases were effective(59.09%),and 4 cases(9.09%)were ineffective,with a total effective rate of 90.91%.5.Pregnancy:3 cases were successfully pregnant during the treatment period,and 18 cases were followed up within half a year,all of which were intrauterine pregnancy,and the pregnancy success rate was 44.68%.6.Comprehensive clinical efficacy:21 cases were cured,19 cases were effective,and 7 cases were ineffective,with a total effective rate of 85.11%.[Conclusion]①Qingluo Enema Decoction combined with magnetic heat and microwave therapy can reduce the levels of cytokines IL-6 and TNF-α in TOI patients with Qi stagnation and blood stasis type.②Qingluo Enema Decoction combined with magnetic heat and microwave therapy can down-regulate MA and whole blood low-shear reduced blood viscosity and up-regulate R value in TOI patients with Qi stagnation and blood stasis type.③ Qingluo Enema Decoction combined with magnetic heat and microwave therapy can significantly improve the total score of TCM symptoms and the total score of local signs in TOI patients with Qi stagnation and blood stasis type.④ Qingluo Enema Decoction combined with magnetic heat and microwave therapy may improve the pelvic environment and promote the absorption of inflammation by reducing the levels of cytokines IL-6 and TNF-α,regulating platelet aggregation rate and blood rheology in TOI patients with qi stagnation and blood stasis type And the release of adhesions,reduce the damage to the fallopian tube,thereby improving the pregnancy rate,worthy of clinical application. |
