| ObjectiveIn this study,fire needle Sihua and Feishu points were used to treat stable chronic obstructive pulmonary disease(COPD).The clinical randomized controlled study method was used to evaluate the self-assessment scale(CAT)and lung of patients with stable COPD before and after treatment.Observation of function,arterial blood gas analysis and serum inflammatory factors and other indicators to evaluate the clinical efficacy of fire acupuncture in the treatment of stable COPD;preliminarily explore the curative effect mechanism of fire acupuncture at Sihua and Feishu points in the treatment of stable COPD,which is the clinical fire acupuncture treatment of stable COPD,COPD provides scientific basis;provides new ideas for clinical treatment of stable COPD.Methods1.Literature researchIn the early stage of this study,a literature review study was conducted to summarize and analyze the pathogenesis,treatment methods,rehabilitation methods,and clinical efficacy of stable chronic obstructive pulmonary disease from both modern medicine and traditional medicine,and to explore effective treatment options and treatments for stable COPD.Research progress.2.Clinical researchThis study is based on the Global Initiative for Chronic Obstructive Lung Disease(GOLD)in 2019 and the Chronic Obstructive Lung Disease in the "Guidelines for the Diagnosis and Treatment of Chronic Obstructive Lung Disease" formulated by the Respiratory Disease Branch of the Chinese Medical Association in 2007 Diagnostic criteria.A total of 60 patients with stable COPD from the Department of Acupuncture and Moxibustion of the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine were screened strictly according to the criteria for inclusion and discharge.They were randomly divided into 30 cases in the fire needle group and 30 cases in the ordinary acupuncture group by using a single blind method.Both groups of patients with stable COPD received conventional treatment(low-flow oxygenation,oral theophylline sustained-release tablets,carbocysteine tablets treatment).2.1 Fire needle group:Main points: Sihua points(Danshu,Geshu),FeishuMatching points: Lung Qi deficiency syndrome with Chize and Taiyuan,Lung Spleen Qi deficiency with Zusanli,Zhongwan,Pishu,Lungshen Qi deficiency with Shenshu,Taixi,Guanyuan,Lung and Kidney Qi and Yin deficiency with Xinshu and Kidney Yu,Guan Yuan,Qi Hai.Twice a week,4 weeks as a course of treatment.2.2 Ordinary acupuncture groupMain points and matching points are same as fire needle group.Once every other day,4 consecutive weeks is a course of treatment.2.3 Curative effect indicatorsBefore and after treatment,by observing the patient’s CAT scale,FEV1,FEV1%,FEV1/FVC,Pa CO2,Pa O2,TNF-α,IL-6 and other indicators,the clinical efficacy of the two regimens in the treatment of stable COPD was evaluated.2.4 Efficacy indexBefore and after acupuncture intervention,the patients’ efficacy before and after treatment was evaluated by cat scale,FEV1,FEV1,fev1/fvc,Pa CO2,Pa O2,tnf-α,IL-6 and other indicators.Results1.Basic informationThere was no significant difference between the two groups of patients in terms of age,gender,course of disease and other baseline data,and there was no statistical significance(P>0.05).2.Statistical results of CAT scores of the two groups of patientsThe two groups of patients underwent CAT assessment before treatment,the second week,and the fourth week.There was no significant difference in CAT scores between the fire acupuncture group(14.20±2.88)and the ordinary acupuncture group(14.60±2.59)before treatment(P>0.05);Compared with before treatment,the CAT scores of the two groups were significantly reduced after treatment,and the difference was statistically significant(P<0.01);compared between the groups,the fire needle group(11.76±2.93)CAT in the second week of treatment The score was significantly lower than that of the ordinary acupuncture group(13.20±2.57),and the difference was statistically significant(P<0.05).In the 4th week of treatment,the CAT score of the fire acupuncture group(7.30±2.68)was significantly lower than that of the ordinary acupuncture group(10.80±2.57),and the difference was statistically significant(P<0.01).3.Comparison of the results of FEV1,FEV/FVC,FEV1% before and after treatment between the two groupsBefore treatment,there was no significant difference in FEV1,FEV/FVC,FEV1%between the two groups of patients(P>0.05).After treatment,FEV1,FEV/FVC,FEV1% in the two groups were significantly higher than before treatment,and the differences were statistically significant(P<0.01 or P<0.05).After treatment,compared with the two groups,the FEV1,FEV/FVC,FEV1% values of the fire needle group were significantly higher than those of the ordinary acupuncture group,and the difference was statistically significant(P<0.01 or P<0.05).4.Comparison of Pa CO2 and Pa O2 results between the two groups before and after treatmentBefore treatment,there was no significant difference in Pa CO2 and Pa O2 between the two groups of patients(P>0.05).After treatment,the Pa CO2 of the two groups was significantly lower than before treatment,and the difference was statistically significant(P<0.01);compared with the ordinary acupuncture group,the fire acupuncture group was significantly lower,and the difference was statistically significant(P<0.01).After treatment,Pa O2 of the two groups was significantly lower than before treatment,and the difference was statistically significant(P<0.01);compared with the ordinary acupuncture group,the fire acupuncture group was significantly higher,and the difference was statistically significant(P<0.01).5.Comparison of the results of TNF-α and IL-6 before and after treatment in the two groupsBefore treatment,there was no significant difference in serum TNF-α and IL-6concentrations between the two groups of patients(P>0.05).Compared with before treatment,the concentrations of TNF-α and IL-6 in the two groups were significantly reduced after treatment,and the differences were statistically significant(P<0.01).After treatment,the concentration levels of TNF-α and IL-6 in the fire-acupuncture group decreased more significantly than those in the ordinary acupuncture group,and the differences were statistically significant(P<0.01).Conclusion1.Choose Sihua points and Feishu points as the main points.Both fire acupuncture and ordinary acupuncture can reduce the CAT score of patients with stable COPD,improve the quality of life of stable COPD,and the effect of fire acupuncture is better than that of ordinary acupuncture.2.Both fire needle therapy and ordinary acupuncture can effectively improve the lung function and arterial blood gas indexes of patients with stable COPD,and fire needle has obvious advantages over ordinary acupuncture.3.Both therapies can reduce the level of serum inflammatory factors in patients with stable COPD,exert anti-inflammatory effects,and improve the lung function and quality of life of patients with stable COPD,and the regulation effect of fire on inflammatory factors is better than ordinary acupuncture.It may be the dual effect of the warm stimulation of fire needle and the stimulation of acupuncture,which reduces the level of inflammatory factors and improves the lung function and quality of life of patients with stable COPD. |
