Objective: To explore the efficacy of leukotriene receptor antagonist montelukast sodium with Mometasone Furoate Nasal Spray on the treatment of adenoid hypertrophy with different severity.Method: 120 children with adenoidal hypertrophy were selected and divided into text group 1 and text group 2 according to degree of nostril by fiberoptic nasopharyngoscope.The grade III hypertrophy was the experimental group 1,Grade IV hypertrophy was the experimental group 2.According to complication,they were divided into simple adenoidal hypertrophy group,secretory otitis media group and allergic rhinitis group.All children were treated with Mometasone Furoate Nasal Spray with leukotriene receptor antagonist montelukast sodium.The levels of IL-2,IL-6 and Cys LTs,percentage change in posterior nostril congestion,symptom scores,adverse reactions incidence.Results:1.Before treatment,clinical symptom score,percentage of posterior nostril congestion,IL-2,IL-6 and Cys LTs levels in text group 2 were significantly higher than those in text group 1(P<0.05).IL-2,IL-6 and Cys LTs in simple adenoid hypertrophy group before treatment were significantly lower than those in secretory otitis media and allergic rhinitis group(P<0.05).IL-2,IL-6 and Cys LTs in each group were significantly higher than in control group(P<0.05).2.At 1 and 2 months after treatment,symptom score,percentage of posterior nostril congestion,IL-2 and IL-6 in IV group were significantly higher than those in text group 1(P<0.05),symptom score,percentage of posterior nostril congestion in simple adenoid hypertrophy group were significantly lower than those in secretory otitis media group and allergic rhinitis group(P<0.05),and symptoms scores in allergic rhinitis group were significantly lower than those in secretory otitis media group(P<0.05).At 1and 2 months after treatment,IL-2 and IL-6 in secretory otitis media group were higher than those in adenoid hypertrophy alone group and allergic rhinitis group(P<0.05).The symptom score,percentage of posterior nostril congestion,IL-2 and IL-6 in each group were significantly lower than those at1 month(P<0.05).3.At 1 month after treatment,Cys LTs in text group 2 was significantly higher than that in text group 1(P<0.05),Cys LTs in secretory otitis media group was significantly higher than that in adenoid hypertrophy alone and allergic rhinitis group(P<0.05).4.There was no significant difference in incidence of adverse reactions between each group(P>0.05).Conclusions:1.The inflammatory factors’ expression in children with adenoid hypertrophy is higher than that in healthy children.The clinical symptoms and pro-inflammatory factors IL-2,IL-6 and Cys LTs’ expression in children are Ⅳmore serious than those in Ⅲchildren.The expression of proinflammatory factors in children with with secretory otitis media or allergic rhinitis was higher than that in children with adenoid hypertrophy alone.2.Montelukast with mometasone furoate can alleviate clinical symptoms children with different severity,reduce inflammatory degree,and it has better effect on children with allergic rhinitis and simple adenoid hypertrophy.3.The safety of Montelukast with mometasone furoate on the treatment of adenoid hypertrophy is high,and safety on children with different severity and complications is almost same.
