| Purpose1.Based on the six meridians model of Zhang Zhongjing’s "Treatise on Cold Damage and Miscellaneous Diseases",the study explores and summarizes the feasibility of the six meridians in the treatment of diabetes and its complications,and attempts to provide an objective and comprehensive interpretation of the various scholars of Chinese medicine in treating this disease from different perspectives.2.The study wants to summarize the clinical thinking of Professor Wu Shentao in treating diabetes and its complications by using the six-meridian pattern identification ideas in depth.And it also tried to explore and summarize the application of the ideas of merging traditional Chinese medicine and modern medicine in type 2 diabetes based on the six-meridian pattern identification ideas.Methods1.Using the keywords "six meridians","six-meridian pattern identification","diabetes mellitus" and "classical formula",the author searchs and organizes the relevant literature,and combined with her own knowledge,then reviews the research overview of the six-meridian pattern identification method and the current situation of modern medical practitioners using the six meridians identification to treat type 2 diabetes mellitus and its complications.2.Through outpatient observation and study of the instructor’s clinical cases of diabetes and its complications,daily consultation with the instructor,study of relevant books recommended by the instructor,as well as combining what the author has learned and thought(especially focusing on the idea of merging traditional Chinese medicine and modern medicine),the author gains a deeper understanding of the relationship between the six meridians and diabetes and its complications,summarizes and compiles the instructor’s clinical experience in using the six-meridian pattern identification method in this disease.Results1.Since the advent of the "Treatise on Cold Damage and Miscellaneous Diseases",many scholars have studied it,and the core content of the six-meridian pattern identification method has been gradually revealed in recent times.All diseases can be treated with the six meridians.However,through searching the literature,we found that not many scholars have used the systematic six-meridian pattern identification method to treat type 2 diabetes mellitus and its complications,and the focus of each scholars is different,so there is a need to further seek common ground while preserving differences and integrate.2.Professor Wu Shentao believes that type 2 diabetes is closely related to the six meridians,with the greater yin and yang brightness axis,which consists of pancreas,stomach and intestines,and muscles,as the core,playing a decisive role in stabilizing blood glucose concentration;closely related to the lesser yang and reverting yin axis,which consists of liver and fat,playing a major regulatory role in stabilizing blood glucose concentration;secondly related to the lesser yin and greater yang axis,which consists of kidney and hypothalamus-pituitary-target gland axis,playing a relatively minor regulatory role in stabilizing blood glucose concentration.It plays a relatively minor regulatory role in stabilizing blood glucose concentration.In terms of treatment,Prof.Wu Shentao advocates the first identification of yin and yang,and then according to the transmission of the six meridians,it can be roughly divided into heat formation and cold formation pathways.The heat formation pathway is from yang brightness(mostly accompanied by symptoms of lesser yang,symptoms of greater yin are often not obvious)to lesser yin heat formation pattern,and then to reverting yin cold-heat complex pattern.The core pathological mechanism is the dry heat of the yang brightness stomach meridian,which leads to excessive food intake and excessive energy in the body.And emotional,stress and other mental factors lead to depression of the Qi of lesser yang liver and gallbladder.Lesser yang and yang brightness’ s depressive heat manifest as disorders of glucose and lipid metabolism.High sugar and high fat stimulation produce glucotoxicity and lipotoxicity,produce tur Bidity,toxicity,heat,phlegm,drinking and other pathological products,leaving the body in a chronic inflammatory state.At this stage,the disease is still in the qi aspect pattern,mostly due to external factors that damage the function of pancreatic islets and produce insulin resistance,leading to the occurrence of this disease.Subsequently,various inflammatory factors activate the coagulation system of the body,and the disease enters lesser yin blood aspect,becoming lesser yin heat formation pattern and produces a pathological product called static blood,while often accompanied by chronic hyperfunction of the hypothalamus-pituitary-target gland axis.Ultimately,when the disease is transmitted to reverting yin,it mostly manifests as dual deficiency of yin and yang,cold-heat complex,serious disorder of endocrine metabolism,and even the phenomenon of yin and yang leaving the decision with sudden high and low blood sugar.To address the core pathological mechanism of this transmission pathway,Prof.Wu formulates his own Hua Zhuo Jie Du Tang to clear lesser yang and yang brightness ’s heat,as well as to clear heat and stasis in lesser yin blood aspect,which is effective in early type 2 diabetes in this transmission pathway.The cold formation pathway is from greater yin(mostly accompanied by symptoms of lesser yang,symptoms of yang brightness are often not obvious)to lesser yin cold formation pattern,and then to reverting yin cold-heat complex pattern.The main pathological mechanism of this transmission pathway is generally due to congenital greater yin spleen deficiency,which is characterized by poor development or storage of pancreatic islets,muscles and fats,leading to islet insufficiency or insulin resistance.In addition,it is often accompanied by stagnation of liver qi in lesser yang.leading to disorders in blood sugar regulation.These reasons eventually lead to the development of this disease.At this time,the disease is still in the qi aspect.After the cold formation of greater yin,the disease passes into lesser yin blood aspect,mostly manifesting as hypochondr IUm-pituitary-target gland axis hypofunction,showing obvious yang deficiency and cold signs.To address the core pathological mechanism of this transmission pathway,Professor Wu’s self-developed Shi Wei Bai Zhu San,which is based on strengthening greater yin and unblocking lesser yang,is highly effective in early type 2 diabetes of this transmission pathway.The treatment of the various chronic complications of type 2 diabetes requires the flexible use of the six meridians theory to select prescriptions and herbs based on the above pathological mechanisms and the characteristics of each diseased visceral tissue.In conclusion,Prof.Wu has developed a complete system for the treatment of type 2 diabetes and its complications in terms of theory,method,prescriptions and medicines. |
