| Objective : Based on the expression of adipogenic related factors,to investigate the efficacy and safety of Tiaozhongjiangtang granules combined with Tiaozhongjiangtang in the treatment of pre-diabetic patients with spleen deficiency and dampness-filled type.Methods:Patients who met the diagnostic criteria of pre-diabetes and TCM syndrome type in line with the wet and full type of spleen deficiency were included in this study,and were grouped according to the random number table method,with 42 patients in each group and a total of 84 patients studied.On the basis of weight loss by routine exercise and healthy diet intervention,the observation group and the acupuncture and medicine combined treatment group were treated with regulating spleen and stomach acupuncture combined with oral Tiaozhongjiangtang granules,in which Tiaozhongjiangtang granules were taken orally 1dose daily,morning and evening,twice a day.The control group was given metformin hydrochloride orally at 0.5g BID,and both groups were given metformin hydrochloride orally for 6 weeks.After the end of treatment,TCM syndrome score was used to evaluate the efficacy of the two groups of patients,and the changes of fasting blood glucose,blood glucose two hours after meal,insulin resistance index,blood lipid,serum Hcy,Cys C,hs-CRP,RBP4 and free fatty acid(FFA)were observed.After 3 months of follow-up after the treatment,the TCM syndrome score,fasting blood glucose,blood glucose two hours after prandial,insulin resistance index,RBP4 and FFA of the two groups of patients were evaluated again,and the results were statistically analyzed.Results:1.Comparison of baseline data between the two groups:Two groups of patients before treatment basic information(sex,age,risk score)there was no statistically significant difference after statistics analysis were comparable,two groups of patients before treatment of TCM symptom score,blood glucose levels,insulin resistance index,blood lipid,serum Hcy,Cys C,hs CRP,RBP4 and FFA difference after statistics analysis was no statistical significance has obvious comparability.2.Comparison of clinical efficacy between the two groups :Found that regulate spleen and stomach after treatment stitch joint adjustable hypoglycemic granule group total effective rate was 87.5% higher than in the metformin group total effective rate was 65%,two groups of clinical curative effect is very obvious difference after statistics analysis and comparison of statistical significance,and not only the curative effect of observation group was better than control group,and from three months follow-up of the observation group curative effect more enduring.3.Comparison of laboratory observation indexes before and after treatment and 3 months follow-up between 2 groups of patients: FBG,2h-BG,HOMA-IR,blood lipid,serum Hcy,Cys C,hs-CRP,RBP4 and FFA in the acupuncture and drug combination group before and after treatment were significantly lower than those in the common western medicine group,with significant difference.4.Comparison of TCM symptom scores between the two groups before and after treatment and 3 months after treatment:The comparison of TCM symptom scores between the two groups before and after treatment showed a significant difference after statistical analysis,and the scores were all decreased after treatment at three time points before and after treatment and follow-up,and the efficacy of acupuncture and drug combination group was better than that of the common western medicine group.5.Safety comparison : After safety evaluation,there was no statistical significance between the two groups,indicating that the two treatment regimens had no obvious adverse reactions and had good safety in clinical application.Conclusions:1.Through this clinical study,it is vertified that the basic pathogenesis of pre-diabetes with spleen deficiency and dampness-filled type is the abnormal movement of spleen and stomach,and the treatment should be based on the principle of regulating spleen and stomach qi and the combination of spleen and stomach acupuncture combined with Tiaozhong Jiangtang granules as the basic treatment.2.Tiaoliang Spleen and Stomachyacupuncture combined with Tiaozhong Jiangtang Granules can significantly improve the blood glucose level,insulin resistance index,blood lipid,serum Hcy,Cys C,hs-CRP,RBP4,FFA and other laboratory indicators in patients with pre-diabetic spleen-deficiency and damping-filled type.Moreover,regulating spleen and stomach acupuncture combined with Tiaozhong Jiangtang granules can effectively improve the TCM syndrome scores of patients.The combination of acupuncture and medicine has a better effect than conventional western medicine and significantly improves the quality of life of patients.3.Tiaoliang Spleen-stomach Acupuncture combined with Tiaozhong Jiangtang Granules has a definite and soft clinical effect,a low safety cost,and a wide application prospect worthy of clinical promotion. |
