| ObjectiveTo observe the therapeutic effect of acupotomy combined with modified syringe suction cup in the treatment of blood stasis type lumbar disc herniation,in order to provide reference for the clinical treatment of the disease.MethodsFrom March 2020 to January 2021,patients in the department of acupuncture and orthopaedics of the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine were included in 70 patients with blood stasis type lumbar disc herniation, including treatment group(n=35)and control group(n=35).The treatment group was treated with acupotomy combined with modified syringe suction cup once every 7 days for 3 times, while the control group was treated with nimesulide tablets 0.1g per time, twice a day for 14 days. The Visual analogue scale(VAS score),Japanese Orthopedic Association score(JOA score)and Roland-Morris disability questionnaire score(RDQ score)of lower back and lower extremities were observed and recorded before and after treatment, and evaluated before and after treatment respectively.Use IBM SPSS 23.0 for statistical analysis, and then draw conclusions.Results1 BaselineThe differences in basic data such as sex, age, body mass index, course of disease,as well as the baseline level of observation indicators such as lower back VAS score, lower extremities VAS score,JOA score and RDQ score between the two groups before treatment were no statistically significant(P>0.05)and were comparable.2 Comparison of results2.1 Lower back VAS scoreThere was statistically significant in lower back VAS score between the two groups before and after treatment(P<0.05),and the lower back VAS score after treatment was lower than those before treatment, indicating that the treatment methods of the two groups could relieve lower back pain. There was no statistically significant in lower back VAS score between the two groups after treatment(P>0.05),and it shows that the two treatments have the same effect on relieving lower back pain.2.2 Lower extremities VAS scoreThere was statistically significant in lower extremities VAS score between the two groups before and after treatment(P<0.05),and the lower extremities VAS score after treatment was lower than those before treatment, indicating that the treatment methods of the two groups could relieve lower extremities pain. After treatment, there was statistically significant in lower extremities VAS score between the two groups(P<0.05),and the treatment group was better than the control group.2.3 JOA scoreThere was statistically significant in JOA score between the two groups before and after treatment(P<0.05),and the JOA score after treatment was higher than those before treatment, indicating that the treatment methods of the two groups could improve the function of lumbar vertebrae. There was statistically significant in JOA score between the two groups after treatment(P<0.05),and the treatment group was better than the control group.2.4 RDQ scoreThere was statistically significant in RDQ score between the two groups before and after treatment(P<0.05),and the RDQ score after treatment was lower than that before treatment, indicating that the treatment methods of the two groups could improve the lower lumbar function. There was statistically significant in RDQ score between the two groups after treatment(P<0.05),and the treatment group was better than the control group.2.5 Overall efficacyIn the treatment group,10 cases were cured,13 cases were markedly effective,8 cases were effective, and 2 cases were ineffective, and the total effective rate was 93.94%.In the control group,4 cases were cured,4 cases were markedly effective,14 cases were effective, and 11 cases were ineffective, and the total effective rate was 66.67%.So the difference in the rank distribution of the efficacy between the two groups was statistically significant(P<0.05).And acupotomy combined with modified syringe suction cup group was better than the control group.Conclusion1 Both acupotomy combined with modified syringe suction cup and oral western medicine are effective in treating blood stasis type lumbar disc herniation.2 Acupotomy combined with modified syringe suction cup has the same curative effect as oral western medicine in relieving lower back pain, but it is more effective in relieving lower limb pain and improving function.3 Acupotomy combined with modified syringe suction cup is effective and feasible in the treatment of blood stasis type lumbar disc herniation, which has certain clinical significance. |
