ObjectiveTo observe the clinical efficacy and adverse reactions of low frequency electrical stimulation combined with Bushen Huoxue Pill in the uterine involution after artificial abortion,Try to explore its mechanism of action.To provide more safe and effective treatment strategies and ideas for poor involution of uterus after induced abortion.MethodsCollected the early pregnancy women who underwent induced abortion at the Tenth Affiliated Hospital of Sun Yat-Sen University from September2019 to September 2020,aged 18-43,had 0-7 previous pregnancies.Approved by the ethics committee of this hospital,380 patients who met the criteria were divided into two groups according to the principle of patient voluntariness,with 237 cases in the experimental group and 143 cases in the control group.Women in both groups were fasted for 4 hours before operation,after intravenous general anesthesia,artificial abortion was performed to terminate pregnancy.Observe 2 hours after operation and give oral antibiotics to prevent infection,intramuscular injection of oxytocin to promote uterine contraction.After operation,The patients in the control group was treated with low frequency electrical stimulation.The patients in the experimental group were given Dingkundan from the first day of operation on the basis of the control group.1pill/time,2times/day.Continue to take until menstruation returns to normal.The abdominal pain,the time of vaginal bleeding and the amount of bleeding were observed in the two groups.Uterine size,endometrial thickness and urinary HCG at 2 weeks after operation were observed too.Determination of serum concentration of HSP70 and HSP90.The menstrual recovery time,menstrual period and menstrual volume were observed and compared with preoperative menstruation.At the same time,the complications such as uterine cavity residue,uterine effusion,irregular menstruation,cervical adhesion and reproductive tract infection were recorded during the follow-up period of the two groups.Results1.General information comparison:The experimental group:18 to 43 years old,the average age was(27.19±5.99)years old.The gestational age ranged from 30 to 70 days,with an average of(49.24±7.85)days.The number of pregnancies was 0 to 7,with an average of(3.82±2.05)times.The number of births was 0 to 4,with an average of(1.95±1.17)times.The number of abortions is 0-6,with an average of(2.79±1.74)times;The menstrual cycle ranges from 23 to 38 days,with an average of(29.76±2.58)days.The menstrual period was 3 to 7 days,with an average of(5.55±1.29)days.The average diameter of gestational sac is(17.22±5.48)mm.The sum of the three diameters of the uterus is(20.63±1.17)cm.The serum concentration of HSP70 is(47.01±14.42)pg/ml;The serum concentration of HSP90 is(79.24±19.085)pg/ml;The control group:18 to 43 years old,the average age was(27.87±5.78)years old.The gestational age ranged from 34 to 70 days,with an average of(48.974 ±7.95)days.The number of pregnancies was 0 to 7,with an average of(3.53±2.08)times.The number of births was 0 to 4,with an average of(2.13±1.08)times.The number of abortions is 0-6,with an average of(2.71±1.92)times;The menstrual cycle ranges from 23 to 38 days,with an average of(30.25±3.06)days.The menstrual period was 3 to 7 days,with an average of(5.47±1.39)days.The average diameter of gestational sac is(16.89±5.72)mm.The sum of the three diameters of the uterus is(20.79 ± 1.19)cm.The serum concentration of HSP70 is(48.42 ±13.61)pg/ml;The serum concentration of HSP90 is(76.13 ±17.138)pg/ml;There was no significant difference in age,gestational age,menstrual cycle,menstrual period,pregnancy,times of delivery and abortion,sac size,uterine size and serum HSP70,90 concentration between the two groups.2.Comparison of the degree and duration of postoperative abdominal pain between the two groups was statistically significant(P<0.05);There was no significant difference in postoperative vaginal bleeding time(days)and bleeding volume(ml).(P<0.05);3.Two weeks after operation,there was significant difference in endometrial thickness(mm)and uterine size(sum of three diameters)by B-ultrasound(P<0.05).There was no significant difference in uterine cavity residue and uterine hemorrhage(P>0.05).4.There was no statistically significant difference in the results of urine pregnancy test 2 weeks after operation(P>0.05);Two weeks after operation,the concentrations of serum HSP70 and HSP90 in the control group were significantly higher than those in the observation group(P<0.05).The concentrations of HSP70 and HSP90 were significantly higher than those before operation(P<0.05).5.Comparison of menstrual recovery:There was significant difference in menstruation recovery time,menstruation period and the amount of menstruation between the two groups(P<0.05).There was significant difference between the recurrent menstruation and the preoperative menstruation in the experimental group(P<0.05).There was no significant difference between the recurrent menstruation and the preoperative menstruation in the control group(P>0.05).Conclusion1.Compared with the low-frequency electric stimulation therapy alone,low-frequency electrical stimulation combined with Bushen Huoxue Pill is more effective in promoting uterine involution after painless induced abortion;2.Low frequency electrical stimulation combined with Bushen Huoxue Pill can significantly relieve postoperative lower abdominal pain and shorten the duration of postoperative abdominal pain.3.The levels of serum HSP70 and HSP90 in the experimental group were significantly lower than those in the control group.This may be related to the mechanism of low frequency electrical stimulation combined with Bushen Huoxue Pill to promote uterine involution.
