Chronic cough is one of the most common clinical symptoms of children’s respiratory diseases.Among the causes of children’s chronic cough,Upper Airway Cough Syndrome(UACS)is second only to Cough Variant Asthma(CVA).Without timely and standardized treatment,the progress of the disease may lead to chronic bronchitis,bronchiectasis,etc.,and even seriously affect the normal life and study of children,and have a serious impact on their physical and mental health.Therefore,by observing the Visual Analog Scale(VAS)scores of four commonly used clinical treatment schemes,namely montelukast sodium,budesonide,nasal irrigation with normal saline,and budesonide combined with nasal irrigation with normal saline for UACS at different times,this study compares the efficacy differences among the four treatment schemes,and provides some reference for clinical treatment of UACS.Objective:To compare the therapeutic effects of montelukast sodium,budesonide,nasal irrigation with normal saline and budesonide combined with nasal irrigation with saline on UACS.Method:This is a prospective study.120 children with UACS who went to the respiratory clinic of the Affiliated Hospital of Chengde Medical College from June 1,2019 to September 30,2020 were selected as the research objects,and were divided into four groups according to random number table:montelukast sodium group,budesonide group,nasal irrigation group(nasal irrigation with normal saline)and combined treatment group(nasal irrigation with budesonide combined with normal saline),with 30 cases in each group.VAS scores were used to quantify the changes of overall symptoms and cough,nasal congestion,runny nose and clear throat before and after treatment,and the VAS scores of children at 0,1,2,3,4,5,6,7 and 8 weeks after treatment were recorded.Univariate analysis of variance,Kruskal-Wails H rank sum test,paired T test,paired Wilcoxon rank sum test and(?)~2test were used to compare the therapeutic effects of montelukast sodium,budesonide,nasal irrigation and budesonide combined with nasal irrigation on UACS.Results:1.Compared with before treatment,the VAS scores of children in the four groups decreased significantly after treatment(P<0.01),and no adverse reactions were found.2.Compared with the VAS scores of montelukast sodium group,budesonide group and nasal irrigation group,the VAS scores of the combined treatment group decreased the most,and the improvement of symptoms was significantly better than that of the other three groups(P<0.05).3.The decrease of VAS score of cough in the combined treatment group was significantly better than that in montelukast sodium group and nasal irrigation group(P<0.001),but there was no significant difference between the other groups(P>0.05).The VAS score of nasal congestion in the combined treatment group was significantly better than that in montelukast sodium group and budesonide group(P<0.05),but there was no significant difference between the other groups(P>0.05).The VAS score of nasal discharge decreased in the combined treatment group,which was better than that of montelukast sodium group and nasal irrigation group(P<0.05),but there was no significant difference between the other groups(P>0.05).The VAS score of throat clearing decreased significantly in the combined treatment group compared with the other three groups(P<0.001),and there was no significant difference between the other groups(P>0.05),as shown in Figure 2.4.The VAS scores of children in montelukast sodium group,budesonide group,nasal irrigation group and combined treatment group tended to be stable at the 7th week,5th week,6th week and 4th week,respectively,with no significant difference(P>0.05).Conclusion:1.Montelukast sodium,budesonide,normal saline nasal irrigation,budesonide combined with nasal irrigation were used to treat UACS children,and the symptoms of children were improved after treatment.2.Compared with montelukast sodium,budesonide and saline nasal irrigation,budesonide combined with saline nasal irrigation has the most obvious improvement in symptoms and the shortest course of treatment for UACS.
