Study On The Medication Rule Of Recurrent Spontaneous Abortion Based On The Theory Of Kidney Dominating Reproduction

Objective: the purpose of this project was to provide reference for TCM clinical treatment of RSA by analyzed the medication rules of TCM in the treatment of RSA with kidney deficiency pathogenesis.Methods: 1.By consulted ancient and modern literature,analyzed the theoretical basis of kidney dominating reproduction.2.Used computer to search the key words “kidney deficiency”and “recurrent spontaneous abortion”in CNKI academic journal database to get the relevant literature,set up the inclusion and exclusion criteria,selected the literature that meet the inclusion criteria,eliminated the literature that meets the exclusion criteria,got the final literature,extracted the literature name,literature source,publication year,first author and corresponding author or tutor,prescription name Then,the processed database was imported into the cloud platform of ancient and modern medical records,and the analysis pool was added.The statistical analysis in the data mining module(frequency analysis of drugs,medicine nature analysis,medicine taste analysis,meridian tropism of drugs)was used Analysis),association analysis(set the confidence level as 0.90,support level as 0.30)and cluster analysis(analyzed the top 20 drugs with frequency,set the distance type as Euclidean distance,clustered method as class average method,displayed with horizontal cluster graph,and take distance > 7 as the boundary)to mine and analyzed the imported data.Results: 1.1 The physiological function of kidney storing essence determines that kidney played a dominant role in human reproductive activities.1.2 The kidney further strengthened the regulation of reproductive activity through the connection with liver,spleen,heart and lung,Qi and blood,Chong and Ren,and uterus.2.1 The general situation of Literature: a total of 143 relevant literature were retrieved,and 92 literature were selected after reading one by one,including 51 journal papers and 41 degree papers.The literature were published from 2009 to2020,and the number of literature showed an overall upward trend.From2017 to 2020,the number of relevant literature was more than 10 every year,and the number of literature in 2020 was the largest,with 18.A total of 99 prescriptions and 109 herbs were extracted.2.2 Syndrome types:kidney deficiency and blood stasis syndrome had been extensively studied,followed by kidney deficiency syndrome,then the spleen and kidney deficiency syndrome,kidney deficiency and liver depression syndrome,Yin deficiency and blood heat syndrome.2.3 Medication: statistical analysis showed that the top five drugs were Cuscuta chinensis,Dipsacus asperata,phellodendron,Angelica sinensis and Salvia miltiorrhiza;the top five drugs were tonifying liver and kidney,strengthening tendons and bones,stabilizing foetus,stopping metrorrhagia and bleeding,and contusion;the top four medicine nature were gentle 34.79%,slight warm 24.28%,warm22.70%,slight cold 13.12%;the top three taste were sweet,bitter,pungent.The top three frequency of channel tropism were liver channel,kidney channel and spleen channel.Cluster analysis showed that there were two kinds of core drugs.The first kind of core drugs were Dipsacus asperata,Cuscuta chinensis,sangsheng and Ejiao.The second kind of core drugs were Angelica sinensis,Salvia miltiorrhiza,Radix Paeoniae Alba,Radix Glycyrrhizae,Eucommia ulmoides,Rhizoma Atractylodis Macrocephalae,Radix Codonopsis,Radix Rehmanniae Preparata,Radix Astragali,Fructus Lycii,Radix Scutellariae,ramie root,Rhizoma Chuanxiong,Poria cocos,Chinese yam and Fructus Corni.According to the correlation analysis of drug combinations,44 results were consistent with confidence ≥ 0.90 and support ≥ 0.30,and the promotion degree of these 44 drug combinations was > 1.The confidence degree of the first nine combinations reached 1,which were Atractylodes macrocephala +Cuscuta,Eucommia ulmoides + Cuscuta,Ejiao,Dipsacus asperata + Cuscuta,Atractylodes macrocephala,Dipsacus asperata + Cuscuta,liquorice,Dipsacus asperata + Cuscuta,Ejiao,sangsheng + Cuscuta,Atractylodes macrocephala and sangsheng +Cuscuta,donkey hide gelatin,mulberry,Dipsacus + Cuscuta.New method combination: dodder,mulberry,Dipsacus,Atractylodes macrocephala,Salvia miltiorrhiza,angelica,Paeonia lactiflora,donkey hide gelatin,licorice.Conclusion: the research on the treatment of recurrent abortion with kidney deficiency in TCM was increasing year by year.The common syndrome types were kidney deficiency and blood stasis syndrome,kidney deficiency syndrome,spleen and kidney deficiency syndrome,kidney deficiency and liver depression syndrome,yin deficiency and blood heat syndrome.The main treatment of recurrent abortion as tonifying kidney and stabilizing fetus,supplemented by supplementing qi and blood,strengthening spleen and soothing liver.The main drugs were tonifying deficiency drugs,followed by promoting blood circulation and removing blood stasis drugs.The medicine natures were mainly gentle,slight warm,warm and slight cold,the medicine tastes were mainly sweet,bitter and pungent,and the meridian tropisms were mainly liver,kidney and spleen.In the process of fetus protection,Shoutai Pill was often used as the basis for dialectical addition and subtraction.New prescription combination:Cuscuta,sangsheng,Dipsacus,angelica,Salvia miltiorrhiza,Radix Paeoniae Alba,Rhizoma Atractylodis Macrocephalae,licorice,maybe could be used as the basic prescription for recurrent abortion of kidney deficiency and blood stasis type.
